AKUT APANDİSİT VE AKUT GASTROENTERİT AYIRIMINDA HEMATOLOJİK PARAMETRELERİN TANI DEĞERİ

Abstract
Giriş: Bu çalışma akut apandisit (AA) ve akut gastroenterit (AGE) ayırımında WBC, nötrofil, lenfosit, kırmızı hücre dağılım genişliği (RDW), ortalama trombosit hacmi (MPV), trombosit ve nötrofil-lenfosit oranını (NLR) içeren hematolojik ve inflamatuvar belirteçlerin tanı değerlerini araştırmak için planlandı. Gereç ve Yöntem: Geriye dönük, olgu kontrollü çalışmada, 1 Ocak ve 31 Aralık 2017 tarihleri arasında 324 AA hastası, 214 AGE hastası ve 127 sağlıklı birey değerlendirildi. Yaş, cinsiyet ve kan örneklerinden elde edilen WBC, nötrofil, lenfosit, RDW, MPV, trombosit ve NLR değerleri gruplar arasında karşılaştırıldı. Bulgular: Akut apandisit ve AGE arasında trombosit, RDW ve MPV değerleri yönünden anlamlı bir farklılık yokken yaş, cinsiyet, WBC, nötrofil, lenfosit, ve NLR değerleri yönünden anlamlı bir farklılık vardı. Hem AA ve kontrol grubu hem de AGE ve kontrol grubu arasında yaş, cinsiyet, WBC, nötrofil, lenfosit, RDW, MPV, trombosit ve NLR değerleri yönünden anlamlı farklılıklar bulundu. Akut apandisit ve AGE ayırıcı tanısında WBC, nötrofil, lenfosit ve NLR için doğruluk oranları sırasıyla %74, % 70, %64 ve %60 olarak bulundu. ROC analizinde WBC, nötrofil, lenfosit ve NLR için eğri altındaki alanlar sırasıyla 0.801, 0.771, 0.636 ve 0.574 olarak bulundu. Sonuç: WBC, nötrofil, lenfosit ve NLR değerlerinin AA ve AGE ayırıcı tanısında klinisyenlere yardımcı olabilir ve değerli bir tanı testi olarak kullanılabilir.
Özet
Introduction: This study was planned to investigate the diagnostic value of hematological and inflammatory markers including WBC, neutrophil, lymphocyte, red cell distribution width (RDW), mean platelet volume (MPV), platelet, and neutrophil-to-lymphocyte ratio (NLR) for distinguishing between acute appendicitis (AA) and acute gastroenteritis (AGE). Material and Method: This retrospective, case-controlled study compared 324 patients with AA, 214 patients with AGE, and 127 healthy individuals between January 1 and December 31, 2017. The age, gender and WBC, neutrophil, lymphocyte, RDW, MPV, platelet, and NLR values from blood samples were compared among the groups. Results: There were significant differences in age, sex, WBC, neutrophil, lymphocyte, and NLR between AA and AGE groups, but no significant difference was found in platelets, RDW and MPV values. Significant differences were found in age, sex, WBC, neutrophil, lymphocyte, RDW, MPV, platelet, and NLR values between AA and control group, and AGE and control group. The diagnostic accuracy rates in the differential diagnosis of AA and AGE were 74% for WBC, 70% for neutrophil, 64% for lymphocyte, and 60% for NLR, respectively. In the receiver operating characteristics (ROC) curve analysis, the areas under the curves (AUCs) for WBC, neutrophil, lymphocyte, and NLR were 0.801, 0.771, 0.636, and 0.574, respectively. Conclusion: WBC, neutrophil, lymphocyte, and NLR may be used as valuable diagnostic tools and may help clinicians in the differential diagnosis of AA and AGE.
