Retrospective evaluation of characteristics and results of our spinal anesthesia practices

Abstract
Objective: Spinal anesthesia provides fast and deep sensorial and motor blockage by administration of small amount of local anesthetics to the subarachnoid space and this technigue is used commonly. In this study, it is aimed to determine how different anesthesia preferences effect the characteristics, results and complications of spinal anesthesia. Material and Method: Between January 2007 and December 2008, all spinal anesthesia administrations performed by the Anesthesiology and Reanimation Department of Uludağ University Medical Faculty were analysed retrospectively. Results: Urological procedures were the most common procedures (%38,8) in our spinal anesthesia practices and as position lateral decubitis (%91,4), as needle type pencil-point needle (%92,8), as size twenty five gauge needle (%84,1), as location L3-4 inter spinous space (%69,2), as local anesthetic hyperbaric bupivacaine (%89,2), and as adjuvant drug fentanyl (%5,2) were the most commonly preferred in our spinal anesthesia techniques. Sensorial blockage was seen in T10 level most commonly. Forty seven (%2.9) patients had general anesthesia after spinal anesthesia: When comparing the variations like needle size, position, premedication and additionial sedation, there was no statistically significant result. Conclusion: The distribition of local anesthetics used in spinal anesthesia in the cerebrospinal liquid is effected by a lot of factors. These factors are generally related with amount of cerebrospinal liquid, dosage of the drug used, side of the enjection, position and conditions of patient. These factors often depend on the preference of the anesthesiologist who will manage the technique. In a conclusion; different spinal anesthesia preferences effect spinal anesthesia characteristics and these preferences may change by time and by new studies.

Amaç: Spinal anestezi, subaraknoid aralığa düşük doz lokal anestezik enjeksiyonu ile hızlı ve derin bir duyu ve motor blok oluşturması nedeniyle klinik olarak sık kullanılan bir tekniktir. Bu çalışmada farklı spinal anestezi yöntemlerinin spinal anestezi karakteristikleri ile sonuçlarının retrospektif olarak değerlendirilmesi ve komplikasyonlarını nasıl etkilediğinin saptanması amaçlanmıştır. Gereç ve Yöntem: Ocak 2007-Aralık 2008- tarihleri arasında Uludağ Üniversitesi Tıp Fakültesi Hastanesi nde uygulanan tüm spinal anestezi bilgileri retrospektif olarak incelendi. Bulgular: Spinal uygulamalarda en sık olarak %38,8 ile ürolojik girişimler, %91,4 lateral dekübitis pozisyonu, %92,8 kalem uçlu iğne, %84,1 25 G spinal iğne, %69,2 L3-L4 aralığı, lokal anestezik ajan olarak %89,2 ile hiperbarik bupivakain, adjuvan ilaç olarak da fentanilin (%5,2) tercih edildiği tespit edildi. En sık T10 seviyesinde duyu bloğu oluştuğu ve 47 (%2,9) hastada genel anesteziye geçildiği görüldü. Genel anesteziye geçilen hastalarda iğne ucu, pozisyon, premedikasyon ve ek sedasyon değişkenleri karşılaştırıldığında anlamlı bir sonuç elde edilemedi. Sonuç: Spinal anestezide kullanılan lokal anestezik ilaçların serebrospinal sıvıda dağılımı pek çok faktör tarafından etkilenmektedir. Bu faktörler sıklıkla serebrospinal sıvının miktarı, kullanılan ilacın dozu ve yoğunluğu, enjeksiyonun yapıldığı taraf, hastanın pozisyonu ve hastaya ait faktörlerdir. Spinal anestezi karakteristiklerini etkileyen bu faktörler daha çok uygulamayı yapan hekimin tercihine bağlıdır. Sonuç olarak farklı anestezi tercihleri spinal anestezi karakteristiklerini etkilemektedir ve bu konuda yapılacak yeni çalışmalara ihtiyaç vardır.
