A RETROSPECTIVE EVALUATION OF CRANIAL COMPUTED TOMOGRAPHY FINDINGS OF THE PATIENTS WITH COMPLAINT OF HEADACHE

Abstract

Introduction: In this study, we aim to retrospectively compare the findings of the patients who were referred to cranial computed tomography (CT) due to headache and to compare the statistics with the literature. Material and Method: In this retrospectively designed study, 1672 patients who applied to our clinic between October 1, 2017 and July 31, 2018 due to headache and also who had cranial CT were evaluated. Patients with a history of cranial trauma, cranial operation and patients younger than 8 years old were excluded, finally a total of 1453 patients were included in the study. Results: A total of 1453 patients (932 women and 521 men) with median age of 44,26 (min: 8, max: 94) were examined. We classified the patients into four groups (8-18 ages, 19-35 ages, 36-60 ages, 61 and older years old). In the first three groups, the most common CT finding was normal cranial CT findings (in order of %76; %74,8; %66,4) and the second one was sinusitis (in order of %22,2; %21; %23,4). The incidence of pathologic findings increased with age and chronic ischemic changes were the most common findings in patients over 61 years of age. The most common cause of headache until the age of 60 was sinusitis. Conclusion: The most common headache cause that can be detected by CT examination in the study population was sinusitis (21.3%), which was assessed by considering the age and gender of the patients. Sinusitis has been observed more frequently in men than in women (29,2% in men and 17% in women). It was seen that the rate of detection of a secondary headache cause in male patients in this study was higher than that of women (46,8% in men and 34,9% in women). This suggests that primary headache was more frequent in women. According to the results which in patients over 61 years of age, the rate of secondary causes increased significantly compared to other age groups. Also in female patients who were 36-60 ages, it has been shown to increase the detection rate of intracranial tumors. According to these findings; applying cross-sectional examination was important in this age group.

Özet

Giriş: Çalışmamızda baş ağrısı şikayeti ile kranial bilgisayarlı tomografi (BT) çekilen hastalarda radyolojik bulguları retrospektif olarak inceleyerek literatürle karşılaştırmayı amaçladık. Gereç ve Yöntem: Retrospektif olarak dizayn edilen bu çalışmada 1 Ekim 2017-31 Temmuz 2018 tarihleri arasında hastanemize baş ağrısı nedeniyle başvuran ve kranial BT tetkiki istenen 1672 hasta değerlendirilmiştir. Travma ve kranial operasyon öyküsü olan hastalar ile 8 yaş altı hastalar çalışma dışında bırakıldıktan sonra toplam 1453 olgu çalışmaya alınmıştır. Bulgular: Çalışmamıza yaş ortalaması 44,26 (min:8, max:94) olan 932 kadın (%64,1) ve 521 (%35,9) erkek toplam 1453 olgu dahil edilmiştir. Hastalarımızı 8-18, 19-35, 36-60 ve 61 yaş üstü olarak 4 gruba ayırdık. İlk 3 grupta en sık normal kranial BT bulguları (sırasıyla %76; %74,8; %66,4) izlenmekte iken ikinci sırada sinüzit (sırasıyla %22,2; %21; %23,4) gelmektedir. BT incelemeye göre patolojik bulguya rastlama oranı yaş ile artmakta ve kronik iskemik değişiklikler 61 yaş ve üstü hastalarda en sık rastlanılan bulgu olarak karşımıza çıkmaktadır. 60 yaşına kadar en sık rastlanılan baş ağrısı nedeni ise sinüzittir. Sonuç: Daha önceki yapılmış çalışmalara kıyasla güncel çalışmamızda daha geniş bir yaş grubu sınıflandırması ve hasta popülasyonu kullanılmış olup hasta yaşı ve cinsiyeti göz ardı edilerek yapılan değerlendirmede, BT inceleme ile tespit edilebilen en sık baş ağrısı nedeni sinüzittir(%21,3). Sinüzit sıklığı erkeklerde kadınlara oranla daha sık olarak gözlenmiştir(erkeklerde %29,2; kadınlarda %17). Baş ağrısı nedeniyle başvuru yapan erkek hastalarda sekonder neden saptanma oranı kadınlara göre daha fazla olduğu görülmektedir(erkeklerde %46,8; kadınlarda %34,9). Bu durum da kadınlarda primer baş ağrısının daha sık olduğunu düşündürmektedir. 61 yaş ve üstü hastalarda elde edilen sonuçlara göre sekonder nedenlerin oranı diğer yaş gruplarına oranla belirgin arttığı, ayrıca 36-60 yaş grubundaki kadın hastalarda ise intrakranial kitle saptanma oranının yükseldiği görülmüştür. Bu bulgular bu yaş grubundaki hastalarda kesitsel incelemeye başvurmanın önemli olduğu düşünülmektedir.
