The effects of different types of anesthesia on fetal İL-6 levels and natural killer cell activity

Abstract

Objective: That they have insufficient immunity, mothers’ immune system have an modulator impact to fetus; obstetrics anesthesia methods used is important. We investigated that the difference between different types of anesthesia on fetal IL-6 levels and NK cell activity in this study. Method: Thirty patients undergoing elective caesarean section were assigned to two groups: In Spinal Anesthesia Group (n:15), anesthesia was performed with hyperbaric bupivacaine and fentanyl. In General Anesthesia Group (n:15), induction was performed with thiopenthal, succinylcholine and sevoflurane. After clamping the umbilical cord, fentanyl and vecuronium were administered. Maternal blood was collected for IL-6 levels at preoperative period and final closure of the skin incision. After neonatal delivery, cord blood was collected for NK cell activity, IL-6 and cortisol levels. Results: The time interval from the beginning of anaesthesia to umblical cord clamping (AbzKkz) was longer in spinal anaesthesia group (p< 0.05) and there was negative correlation between this time and fetal IL-6 levels (p< 0.05). Pre- postoperative maternal and fetal IL-6 levels were not influenced by anesthesia techniques (p> 0.05). Fetal cortisol levels were higher in general anesthesia group (p< 0.05). There was no difference between pre-and postoperative maternal IL-6 levels within groups (p> 0.05). NK cell activity was smilar in both groups (p> 0.05). There was negative correlation between NK cell activity and fetal IL-6 levels in general anesthesia group (p< 0.05). Conclusion: Longer AbzKkz and its negatively correlation with fetal IL-6 level in spinal anesthesia group and higher fetal cortisol level in general anesthesia group may be important for fetal defense to stress and infection.

Özet

Amaç: Yetersi: immüniteye sahip olduklarından, anneye ail immiin sistemin modulator bir etkiye sahip olduğu fetüs için; obstetrikte kullanılan anestezi yöntemleri ö/ıenı taşımaktadır. Bu çalışmada: iki farklı anestezi yönteminde, fetal IL-6 düzeyleri ve NK hücre aktivitesi ııiıı etkilenmesi açısından fark olup olmadığı araştırılmıştır. Yöntem: Elektif sezeryan planlanan 30 lıasia iki gruba ayrıldı: Spinal Anestezi Grubu (n;l5)'nda; lıiperbarik bupivakain ve fentanil He anestezi uygulandı. Genel Anestezi Grubu (n:15)'ııda, anestezi indüksiyonıı liyopental. süksinilkolin ve sevofluran ile yapıldı. Kordon klemplenmesinden sonra fentanil ve veküronyum, eklemli. Annelerde IL-6 seviyelerine operasyon öncesinde ve cill kapatılırken bakıldı. Yenidoğaııda ise: doğduktan sonra IL-6 seviyesi. NK hücre aktivitesi ve kortizol düzeylerine bakıldı. Bulgular: Spinal anestezi grubunda, anestezi başlama zamanı ile kordon klemplenmcsine kadar geçen süre (AbzKkz) uzun bulunmuştur (p 0,05i. Fetal kortizol düzeyleri, genel anestezi grubunda daha yüksektir (p 0.05). NK hücre aktivitesi açısından her iki grup benzer bulunmuştur 0.05). NK hiicre aktivitesi ile fetal IL-6 düzeyleri arasında, genel anestezi grubunda, anlamlı negatif bir ilişki bulunmuştur (p< 0,05). Sonuç: Spinal anestezi grubunda; AbzKkz tun uzunluğu ve bunun IL-6 ile negatif korelasyonu ve genel anestezi grubundaki yüksek fetal kortizol düzeyi: fetıısun iııfeksiyon ve strese karşı savunması için önemli olabilir.
