Cystic Macular Edema Secondary to Prostaglandin Analog

Abstract

Cystoid macular edema (CME) can develop due to a variety of causes. It is also known that some medications can cause CME. Antiglacomatous agents from the group of prostaglandin analogues are among of these medications. A-82 year-old man with primary open angle glaucoma followed in our glaucoma department developed vision loss 3 months after the addition of a prostaglandin analog agent to his current anti-glaucoma treatment. An epiretinal membrane (ERM) and CME was diagnosed after optical coherence tomographic investigation. Although established ERM could be accused for CME, prostaglandin analog was discontinued because of development of visual symptoms 3 months after starting the prostaglandin analog, and his vision started to improve with recovery of CME beginning 1 week after discontinuation. Our case was presented here, because CME might be treated in other ways in case of failure to correct evaluation of the clinical picture and its importance for this reason.

Özet

Kistik makula ödemi (KMÖ) bir çok nedene bağlı gelişebilmektedir. Kullanılan bazı ilaçların da KMÖ'ye neden olabildiği bilinmektedir. Bu ilaçlardan birisi de prostaglandin analogu antiglokomatöz ajanlardır. Glokom birimimizde takip edilen 82 yaşındaki erkek olgu mevcut antiglokomatöz tedavisine ilave edilen prostaglandin analogu kullanımından 3 ay sonra başlayan görme keskinliği azalmasından yakınmış ve yapılan incelemede epiretinal membran ve kistik makula ödemi saptanmıştır. Saptanan KMÖ, eşlik eden ERM'na bağlanabilecek iken olgunun semptomunun prostaglandin analogu başlanmasından 3 ay sonra gelişmesi nedeni ile prostaglandin analogu kesilmiş ve 1. haftadan itibaren hastanın görmesi düzelmeye KMÖ ise kaybolmaya başlamıştır. Olgumuz, doğru değerlendirilememesi durumunda KMÖ'nin başka yollar ile tedavisine yol açabileceği ve bu nedenle önemli olduğu düşüncesi ile burada sunulmuştur.
