THE RELATIONSHIP BETWEEN ADENOID HYPERTROPHY AND ALLERGIC RHINITIS: IS ADENOID VOLUME EFFECTIVE?

Abstract

Objective: Adenoid hypertrophy may cause significant morbidity in children, but its association with allergic rhinitis is not clearly understood. The aim of this study is to determine the Visual Analog Scale and Total Nasal Sypmtom Scores which are used in the diagnosis and treatment folllow- up of allergic rhinitis, in children with adenoid hypertrophy. Material and Method: 209 children (144 adenoid hypertrophy group, 65 control group) were included in the study. Children's runny nose, nasal congestion, sneezing, itchy eye, postnasal discharge complaints were evaluated using the Total Nasal Symptom Score and allergic rhinitis related symptoms were evaluated using the Visual Analog Score. Results: There was a statistically significant difference between Total Nasal Sypmtom Scores and Visual Analog Scale scores between children with and without adenoid hypertrophy (p <0.001). Positive correlation between adenoid hypertrophy and VAS (p: 0.86; p <0.001) and TNSS (p: 0.83; p <0.001) scores were detected. Conclusion: In this study, it is concluded that the severity of allergic rhinitis symptoms increases as adenoid volume increases in patients with adenoid hypertrophy. Treatment success in patients with adenoid hypertrophy can be increased by evaluating the patients from the point of allergic rhinitis in conjunction with pediatric allergy specialists.

Amaç: Adenoid hipertrofisi, çocuklarda önemli morbiditeye neden olabilir, ancak alerjik rinit (AR) ile ilişkisi net olarak anlaşılabilmiş değildir. Adenoid hipertrofisi olan çocuklarda, alerjik rinit tanı ve tedavi takibinde kullanılan Toplam Nazal Semptom Skoru ve Vizüel Analog Skor değerlerinin belirlenmesi amaçlanmıştır. Yöntem ve Gereçler: Çal ışmaya 209 (144 adenoid hipertrofili grup, 65 kontrol grubu) çocuk dahil edildi. Çocukların burun akıntısı, burun tıkanıklığı, hapş ırma, göz kaşıntısı, postnazal akıntı şikayetleri Toplam Nazal Semptom Skoru kullanılarak, alerjik rinit ile ilgili semptomları Vizüel Analog Skor kullanılarak değerlendirildi. Bulgular: Adenoid hipertrofisi olan ve olmayan çocuklar arasında Toplam Nazal Semptom Skoru ve Vizüel Analog Skor aras ında istatiksel olarak anlamlı farklılık saptandı (p < 0.001). Adenoid hipertrofisi ile Vizüel Analog Skor (p: 0.86; p<0.001 ) ve Toplam Nazal Semptom Skoru (p: 0.83; p<0.001) skorları arasında pozitif korelasyon tespit edildi. Sonuç: Bu çalışmada, adenoid hipertrofisi olan hastalarda adenoid volümü arttıkça alerjik rinit semptomlarının şiddetinin de arttığı belirlenmiştir. Adenoid hipertrofisi olan hastalarda tedavi başarısı, alerjik rinit açısından çocuk alerji uzmanları ile birlikte değerlendirilerek arttırılabilir.
