A Neonatal Breast Abscess Due to Pseudomonas aeruginosa: Case Report

Abstract
Mastitis and breast abscess are not seen frequently during neonatal period. The most common pathogen is Staphylococcus aureus. Gram negative species are rarely seen. With surgical drainage, and proper antibiotherapy complete resolution is attainable. If not treated properly, severe suppurative complications may develop. A 40-day-old infant with complications of redness, swelling and warmth on the left breast that started 15 days ago were evaluated, and treated with the diagnosis of breast abscess in an external center, however the disease did not regress, so she was hospitalized in our service. Surgical drainage was performed for the breast abscess which was shown on ultrasound. Pseudomonas aeruginosa was isolated from culture of the abscess material , and appropriate antibiotherapy was adjusted according to the antibiogram results. In this article, a case of breast abscess which is remarkable for occurring in the neonatal period and to be caused by Pseudomonas aeruginosa was presented with a review of recent literature.
Özet
Mastit ve meme apsesi neonatal dönemde sık görülmemektedir. En sık etken Staphylococcus aureus’tur. Gram negatif etkenler nadir görülür. Cerrahi drenaj ve uygun antibiyotik tedavisi ile tam iyileşme sağlanabilir. Uygun tedavi yapılmadığı durumlarda ciddi süpüratif komplikasyonlar meydana gelebilir.Postnatal 40 günlük olgu, 15 gün önce başlayan sol memede kızarıklık, şişlik ve ısı artışı yakınmaları ile dış merkezde değerlendirilmiş ve meme apsesi tanısı ile tedavi görmüş olmasına rağmen, yakınmalarında gerileme olmaması nedeniyle servisimize yatırıldı. Ultrasonda apse görünümü olan hastaya cerrahi drenaj uygulandı. Apse kültüründe Pseudomanas aeruginosa üremesi olan hastanın antibiyotik tedavisi antibiyograma uygun şekilde düzenlendi. Tedavi sonrası hastada tam iyileşme görüldü. Bu makalede, neonatal dönemde meydana gelmesi ve Pseudomanas aeruginosa gibi nadir bir etkeni olması nedeniyle dikkat çekici olan meme apsesi olgusu literatür bilgileri gözden geçirilerek sunulmuştur
