Cervicofacial lymph node enlargement in childhood

Abstract

Aim: To determine the clinical and laboratory findings of benign and malignant causes of cervicofacial lymphadenopathies in children. Methods: In this prospective study, we evaluated 40 children who had been admitted to our Pediatric Oncology and Hematology Department because of enlarged cervicofasial lymph nodes. The patients' age and sex, location, duration, size, mobility and extension of lymph nodes, associated local and systemic symptoms, diagnostic laboratory or radiologic investigations and final diagnosis were determined. Features of lymph nodes were compared between patients with malignant and benign diagnosis. Results: A specific cause of lymphadenopathy was not documented in 21 (52.5 %) cases. Eight (20 %) cases were classified as having a malignant disease and 11 (27.5 %) cases were diagnosed as having an infectious disease causing lymphadenopathy. Location (supraclavicular and posterior cervical), duration (>4 weeks), size (>3 cm) and mobility (fixed) of lymphadenopathy, abnormalities in chest X-ray and abdominal ultrasonography and the presence of weight loss and night sweats were determined as being important to alert the pediatrician about the probability of malignancy. Conclusion: It is not always possible to identify a specific cause of cervicofacial lymphadenopathy in childhood. The presence of supraclavicular and fixed lymphadenopathy, mediastinal and abdominal abnormalities and the presence of weight loss were determined as being the most important parameters for presumptive diagnosis of malignant disease.

Amaç: Çocuklarda benign ve malign nedenlere bağlı servikofasiyal lenfadenopatilerin klinik ve laboratuvar bulgularını belirlemek. Yöntem: Bu prospektif çalışmada servikofasiyal lenf nodu büyümesi nedeniyle pediatrik onkoloji ve hematoloji kliniğine başvuran 40 çocuk değerlendirildi. Hastaların yaş ve cinsiyetleri, lenf nodlarının lokalizasyon, süre, boyut, hareketlilik ve yayılımı ile eşlik eden lokal ve sistemik semptomları, tanısal laboratuvar ve radyolojik çalışmaları ve kesin tanıları belirlendi. Malign ve benign tanılı hastaların lenf nodlarının özellikleri karşılaştırıldı. Bulgular: Yirmi bir (% 52.5) vakada lenfadenopatinin spesifik nedeni belirlenemedi. Sekiz (% 20) vaka malign hastalığa bağlı ve 11 (% 27.5) vaka da infeksiyon nedenli lenfadenopati olarak gruplandırıldı. Lenfadenopatinin lokalizasyonu (supraklavikular ve posterior servikal), süresi (> 4 hafta), boyutu (>3 cm) ve hareketliliği (fikse), akciğer grafisi ve batın ultrasonografisinde anormallik ve kilo kaybı ve gece terlemesinin varlığı, malignite olasılığı yönünden uyarıcı bulundu. Sonuç: Çocukluk çağında servikofasiyal lenfadenopatinin spesifik nedenini belirlemek her zaman mümkün değildir. Supraklavikular lokalizasyonlu ve fikse lenfadenopati varlığının, mediastinal ve abdominal anormallikler ve kilo kaybının bulunmasının malignite ön tanısında en önemli parametreler olduğu belirlendi.
