Abstract

The novel type coronavirus infection (COVID-19) is generally detected in adults, but it can be also seen in childhood and newborn period. Although the data on the transition from pregnancy to fetus in newborns is still insufficient, newborns are at risk due to their immature immune systems, the possibility of perinatal contact in vaginal deliveries and postpartum contact from asymptomatic mothers. Case report definition is similar to that used for the adult group. From the birth of a child from suspected/positive pregnant women, neonatal precautions should be taken and under appropriate birth conditions, safe transport should be provided by isolation precautions. Infants should be isolated in neonatal intensive care units, and standard and droplet infection precautions should be taken, since the main route of transmission is through contact and droplets. This disease does not have specific clinical features, so supportive treatment of this infection is applied according to the treatment requirements of the patients. In this process, after the mother is informed about the supply of breast milk, the mother can breastfeed her baby with a medical/surgical mask after hand hygiene, or by providing hand hygiene and nipple cleaning, mother can milk to bottle so that the baby can receive breast milk. Comprehensive studies are needed on vertical/horizontal transmission, follow-up and treatment of COVID-19 infection and also short and long-term outcomes of this infection.

Özet

Yeni tip koronavirus enfeksiyonu (COVID-19) daha çok erişkinlerde görülmekle birlikte, çocukluk yaş grubunda ve yenidoğan döneminde de görülebilmektedir. Yenidoğanlarda gebeden fetüse vertikal geçiş konusunda veriler hâlen yetersiz olmakla birlikte, yenidoğanlar immun sistemlerinin immatür olması, vajinal doğumlarda perinatal temas olasılığının olması, asemptomatik annelerden doğum sonrası temas nedeni ile risk altındadırlar. Yenidoğanda olgu tanımında erişkin yaş grubu tanımlaması kullanılmaktadır. kuşkulu/pozitif gebelerin doğumundan itibaren yenidoğana ait önlemler alınmalı, uygun doğum koşullarında izolasyon önlemleri alınarak güvenli transport sağlanmalıdır. Bebekler yenidoğan yoğun bakım ünitelerinde izole edilerek temel bulaş yolu temas ve damlacık yolu ile olması nedeni ile standart ve damlacık enfeksiyonu önlemleri alınmalıdır. Özgül bir klinik tablosu olmayan bu enfeksiyonun tedavisinde uygulanacak destek tedavi gereksinimlere göre uygulanmaktadır. Bu süreçte anneye anne sütünün temini hakkında bilgilendirme yapılarak anne bebeğini el hijyeni sağlandıktan sonra tıbbi/cerrahi maske ile emzirebilir veya yine el hijyeni ve meme başı temizliğini sağlayarak maske ile sütünü sağıp bebeğin anne sütü almasını sağlayabilir. COVID-19 enfeksiyonun vertikal veya horizontal geçişi, izlem ve tedavisi, kısa ve uzun dönem sonuçları hakkında kapsamlı çalışmalara gereksinimi vardır.
