THE EFFECTS OF LOKAL APPLICATION OF LIDOCAINE AND BUPIVACAINE ON POSTOPERATIVE ANALGESIA IN SEPTOPLASTY PATIENTS

Abstract

Introduction: Preoperative local anesthetic and adrenaline application decrease surgical field bleeding and improves postoperative analgesia. The aim of this study was to compare the effects of bupivacaine and lidocaine anesthesia on peroperative bleeding, hemodynamics, postoperative pain, and analgesic consumption. Material and Methods: The study included 60 patients, 35 in Lidocaine +Adrenaline (LA) and 25 in Bupivacaine + Adrenaline (BA) group. After anesthesia induction the patients were done submucoperichondrial local anesthesia with either 10 ml of 1% lidocaine+0.00625mg/ml adrenaline in LA group, or 10 ml of 0.25% bupivakain+0.00625mg/ml adrenaline solution in BA group. Patientcontrolled analgesia with tramadol was commenced on in all patients in Recovery Room. Vital signs, Visual Analog Scale (VAS) scores, Ramsay sedation scale scores were recorded on postoperative 30th minute, 3, 6, 12 and 24th hours. The amount of tramadol used was also recorded. Surgical bleeding amount were evaluated with 5 graded scale by the surgeon. Results: The amount of analgesics and sleep quality scores were not different between the groups. The patient satisfaction was statistically higher in BA group (P=0.031). The postoperative VAS scores on 6th and 12th h were significantly decreased in BA group when compared with LA group. Conclusion: This study concluded that preoperative local anesthesia with bupivacaine is more effective than lidocaine in decreasing postoperative pain in septoplasty patients

Özet

Giriş ve Amaç: Septoplasti ameliyatlarında preoperatif uygulanan lokal anestezik-adrenalin solüsyonu vazokonstriksiyon etkisi ile kanamayı azaltır ve postoperatif analjeziye katkı sağlar. Bu çalışmada septoplasti hastalarında lidokain-adrenalin ve bupivakain-adrenalin anestezisinin hemodinami, kanama ve postoperatif ağrı düzeyleri ile analjezik tüketimi üzerine etkilerinin karşılaştırılması amaçlandı. Gereç ve Yöntem: Çalışmaya 35"i Lidokaine + Adrenalin (LA) grubunda, 25"i Bupivakaine + Adrenalin (BA) grubunda olmak üzere 60 hasta katılmıştır. Anestezi indüksiyonun ardından cerrahi başlamadan önce LA grubundaki hastalara 10 ml %1 lidokain+0.00625mg/ml adrenalin solüsyonu ile, BA grubundaki hastalara ise 10 ml %0.25 bupivakain+0.00625mg/ml adrenalin solüsyonu ile submukoperikondrial alana dental enjektör kullanılarak topikal anestezi uygulandı. Derlenme odas ında tüm hastalara tramadol ile hasta kontrollü analjezi başlandı. Postoperatif 30. dakikada ve 3, 6, 12. ve 24 saatlerde vital bulgular, Visual Analog Scale (VAS) ile ağrı düzeyleri ve Ramsay sedasyon skalası ile sedasyon düzeyleri değerlendirildi. Kullanılan tramadol miktarı, ilk analjezik ihtiyacına kadar geçen süre ve hasta memnuniyeti kaydedildi. Cerrahi kanama miktarı cerrah tarafından 5 basamaklı skala ile değerlendirildi. Bulgular ve tartışma: Postoperatif ek analjezik kullanımı ve uyku kalitesi skoru her iki grupta da benzerdi. Hasta memnuniyeti BA grubunda anlamlı derecede yüksekti (p=0.031). BA grubunda postoperatif 6. (p=0.013) ve 12. (p=0.040) saatlerde VAS skoru LA grubuna göre anlamlı derecede düşük bulundu. Sonuç: Bu çalışma septoplasti uygulanan hastalarda bupivakain ile preoperatif lokal anestezi yapılmasının lidokaine göre postoperatif ağrıyı daha çok azalttığını göstermiştir.
