Abstract
Introduction: Primary central nervous system lymphomas constitute 1% of all brain tumors and 4-6% of extranodal lymphomas. Interventions performed with neuronavigation in deeply located lesions have led to an increase in the diagnosis of PSSSL in recent years. Material and methods: Patients diagnosed with PSSSL in our clinic between 2013-2019 were analyzed retrospectively. The demographic characteristics of the patients, the location of the mass, the surgical procedure, the treatment modalities given to the patients by the hematology and the prognosis of the patients were evaluated. Results: A total of 13 patients were diagnosed with primary central nervous system lymphoma. The average age of the patients was 64. 8 of the patients were female and 5 were male. When the first complaints of the patients were evaluated, the most common complaint was failure to walk. Parenchymal involvement was observed in all patients. While 3 patients (23%) had solitary lesions, 10 patients had multiple (77%) involvement. 4 of the patients underwent total excision, 8 of them underwent diagnostic biopsy accompanied by neuronavigation.The pathology result of all 13 patients was compatible with diffuse large B cell lymphoma. Conclusion: As a result of advances in surgical assistive technologies such as stereotaxy and navigation for diagnostic biopsy, the number of PSSSL has increased significantly in recent years. We present the results of cases with a pathological diagnosis PSSSL that was operated with cranial mass.

Giriş: Primer santral sinir sistemi lenfomaları tüm beyin tümörlerinin %1’ini, ekstranodal lenfomaların ise %4-6’sını oluşturur. Derin yerleşimli lezyonlara nöronavigasyon eşliğinde yapılan girişimler son yıllarda PSSSL tanısında artışa neden olmuştur. Gereç ve Yöntem: 2013-2019 yılları arasında kliniğimizde PSSSL tanısı konan hastalar retrospektif olarak incelendi. Hastaların demografik özellikleri, kitlenin yerleşimi, cerrahi prosedür, cerrahi sonrası hastalara hematoloji tarafından verilen tedavi modaliteleri ve hastaların prognozu değerlendirildi. Bulgular: Toplam 13 hastaya primer santral sinir sistemi lenfoması tanısı konulmuştur. Hastalardan 8’i kadın 5’i erkek, yaş ortalamaları 64’tür. Hastaların ilk başvuru şikayetleri değerlendirildiğinde en sık şikayet yürüyememe, dengesizlikti. Hastaların tümünde parankimal tutulum izlendi. 3 hastada (%23) soliter lezyon saptanırken, 10hastada multipl (%77) tutulum mevcuttu. Hastalardan 4’ üne total eksizyon, 8’ ine ise tanı amaçlı nöronavigasyon eşliğinde biopsi uygulandı. 13 hastanın tümünde patoloji sonucu diffüz büyük B hücreli lenfoma ile uyumlu geldi. Sonuç: Tanısal biopsi için stereotaksi ve navigasyon gibi cerrahi yardımcı teknolojilerdeki gelişmeler neticesinde son yıllarda PSSSL sayısında belirgin artış görülmektedir. Kranial kitle tanısı ile opere edilip patolojik tanısıPSSSL olan vakaları değerlendirilerek sonuçlarını sunuyoruz.
