The Comparison of Adjustable Single-incision Mini-Sling and Transobturator Tape for the Treatment of Stress Urinary Incontinence

Abstract
Objective: The aim of the study was to compare the efficacy and complications of adjustable single-incision mini-sling (A-SIMS) with transobturator tape (TOT) in surgical management of female urinary incontinence. Methods: The results of 54 patients performed A-SIMS and TOT were evaluated retrospectively. Inclusion criteria were stress urinary incontinence with valsava leak point pressure <60 cm H2O and at least one-year follow-up. Exclusion criteria were pelvic organ prolapsus, concominant or previous genitourinary surgery and patients without urodynamic assessment. Patients were enrolled into two groups as A-SIMS and TOT, each group included 27 patients. Results of the operations (postoperative hemoglobin decrease, operation time, perioperative complication, urinary retention, postoperative pain) and efficacy of the surgery (objective cure rate, subjective cure rate, failure rate) were compared. Results: Both of the groups were similar according to the patients characteristics. The mean follow-up period was 21.5 and 17.7 months in TOT and A-SIMS groups, respectively. The difference between the two groups according to objective cure rate, subjective cure rate and failure rate was not statistically significant. Postoperative hemoglobin decrease and operation time in the A-SIMS group were significantly lower than in the TOT group. Five patients had postoperative pain in the TOT group. However, no pain was revealed in the A-SIMS group. Besides, no perioperative complication was revealed in both of the groups. Conclusion: In short-term period, A-SIMS is as effective and safe as TOT in the surgical management of female urinary incontinence. However, A-SIMS may be superior as a simple procedure having shorter operation time.

Amaç: Stres üriner inkontinans şikayetinde adjustable single-incision mini-sling (A-SİMS) ve transobturator bant (TOT) sık ve güvenle kullanılan cerrahi işlemlerdir. Her iki yöntemin birbirine üstünlüğü hala tartışılmaktadır. Çalışmamızda TOT ve A-SİMS uygulamalarının etkinlik ve komplikasyonlarını karşılaştırmayı amaçladık. Yöntemler: TOT ve A-SİMS uygulanan 54 hastanın sonuçları retrospektif olarak değerlendirildi. Yirmi yedi hasta TOT, 27 hasta da A-SİMS grubunu oluşturdu. Hastalara ait, vücut kitle indeksi, parite ve zor doğum hikayelerine ait bilgiler ile operasyona ait operasyon süresi, preoperatif ve postoperatif hemoglobin değerleri, operasyon ve yatış süreleri, perioperatif komplikasyon (üriner retansiyon, postoperatif ağrı) son olarak da tedavinin etkinliği (objektif tedavi oranı, sübjektif tedavi oranı, yetersiz tedavi oranı) belirlendi. Elde edilen veriler birbiri ile karşılaştırıldı. Bulgular: Çalışmada TOT yapılan 27 hasta ve A-SİMS yapılan 27 hasta olmak üzere toplam 54 hastaya ait veriler değerlendirildi. Operasyon süresinin (p<0,001) ve postoperatif hemoglobindeki düşüşün (p=0,007) A-SİMS grubunda anlamlı olarak az olduğu görüldü. TOT grubunda 5 hastada görülen periyoperatif ağrıya karşılık A-SİMS grubunda ağrı saptanmaması istatistiksel olarak da anlamlıydı (p=0,019). Ortalama takip süresi TOT grubunda 21,5 ay, A-SİMS grubunda 17,7 ay olarak bulundu. Objektif tedavi, sübjektif tedavi ve yetersiz tedavi oranlarına bakıldığında iki grup arasındaki farkın istatistiksel olarak anlamlı olmadığı görüldü. Sonuç: Her ne kadar A-SİMS prosedürünün daha az invaziv olması ve de hastane yatış süresinin kısa olması gibi avantajlar görülse de cerrahi işlem ve tipi seçiminde operatörün deneyimine ek olarak hasta ile ilgili risklerin tartışılması sonucu karar verilmesi kanaatindeyiz.
