Late-Onset Pneumothorax Developed in a Tracheostomy Patient

Abstract

Tracheostomy is a frequently used method for the patients with prolonged endotracheal intubation, and mechanical ventilation so as to decrease the parametres concerning patient care, and infection. Percutaneous tracheostomy (PDT) is a preferred method in that it has lower complication rates, and shorter bed-side procedural time relative to surgical tracheostomy. Still it is an invasive procedure where generally mild complications may develop, however some some life threatening complications like pneumothorax and pneumomediastinum have been reported as well. In this case presentation we examined pneumothorax developed in a patient two days after uncomplicated tracheostomy with the Griggs method. Our patient was a 63 –year- old female weighing 92 kg had been operated with the diagnosis of atherosclerotic heart disease. For the patient without any surgical problem, percutaneous tracheostomy was decided to be performed because of prolonged intubation, and so she was transferred to our clinic. Bronchoscopy assisted PDT with Griggs technique was performed. However, subcutaneous emphysema, and pneumothorax were observed within 48 hours, so a chest tube was inserted under local anesthesia. Tracheostomy cannula was drawn and the patient was intubated again using an endotracheal tube with resultant improvement in her oxygen saturation. However 2 days later the patient died because of a cardiac arrest. In many studies, superiorities of percutaneous tracheostomy have been revealed relative to surgical method. But like all invasive procedures PDT is not innocent, and one of the most serious complication to be developed is pneumothorax. Although it occurs in relatively late period, pneumothorax should be kept in mind and necessary treatment and interventions should be initiated immediately.

Endotrakeal entübasyon ve mekanik ventilasyon uygulamasının uzadığı hastalarda hasta bakımı ve infeksiyon parametrelerinin azaltılması açısından trakeostomi açılması sık kullanılan bir yöntemdir. Perkutan trakeostomi (PDT) cerrahi trakeostomi yöntemine kıyasla komplikasyon oranının düşük olması ve hasta başında kısa sürede uygulanabilmesi nedeni ile yeğlenen bir yöntem olmaktadır. Yine de invaziv bir girişim olan PDT ile gelişebilen komplikasyonlar genelde hafif olmasına rağmen, bazen pnömotoraks, pnömomediastinum gibi yaşamı tehdit eden komplikasyonlar da bildirilmiştir. Bu olgu sunumunda, Griggs yontemi ile komplikasyonsuz açılan trakeostomiden 2 gün sonra gelişen pnömotoraks olgusunu inceledik. Hastamız, 63 yaşında 92 kilo kadın hasta, aterosklerotik kalp hastalığı tanısı ile opere edilmişti. Ameliyat yö- nünden herhangi bir sıkıntısı olmayan hastaya uzamış entübasyon nedeni ile perkutan trakeostomi açılmasına karar verilerek kliniğimize devir alındı. Bronkoskopi eşliginde Griggs tekniği ile trakeostomi açılan hastada, 48 saat sonra subkutan amfizem ve pnömotoraks gözlendiğinden lokal anestezi altında toraks tüpü takıldı. Trakeostomi kanülü çekilip endotrakeal tüp ile yeniden entübe edilen hastada toraks tüpü takıldıktan sonra oksijen saturasyonu düzeldi. Ancak hasta 2 gün sonra kardiyak arrest nedeni ile ex oldu. Perkutan trakeostominin cerrahi yönteme göre üstünlükleri birçok çalışmada ortaya konulmuştur. Ancak bütün invazif girişimler gibi PDT'de tamamen masum değildir ve gelişebilecek en ciddi komplikasyonlardan biri de pnömotorakstır. Oranla geç dönem olmasına rağmen, pnömotoraks olasılığı perkutan trakeostomide akılda tutulmalı ve gerekli tedavi ve müdahale başlanılmalıdır.
