Comparison of the effects of sevoflurane and desflurane on hemodynamic, recovery and postoperative side effects in adult patients who had sinus and nasal surgery

Abstract

Background: While controlled hypotension is important to improve the surgical comfort and better the field of view at the operation side, rapid awakening and recovery is also important to have a fast control of the airway and follow the surgical complications of the patient who has sinus or nasal operation. In this study we aimed at comparing the effects of sevoflurane and desflurane in perioperative hemodynamics, postoperative recovery, and the side effects on perioperative hemodynamics, at postoperative recovery and side effects of sevoflurane and desflurane. Material and Methods: Forty patients undergoing sinus and nasal operation were randomly divided into two groups. One of the groups was given sevoflurane (Group S) and the other group desflurane (Group D). General anesthesia was applied for both groups and remifentanil infusion was used as analgesia. General anesthesia was done by using these gases also remifentanil infusion given for analgesia to each group. After the surgery, scores of tracheal extubation, eye opening, obeying the comments and recovery time were noted. Results: There were no differences regarding demographic data between two groups. The MAP values in group D was lower than the other group in intraoperative periods of 15.min, 40.min, 45.min, 50.min, 55.min, 60.min, and 65 minutes respectively. The MAC values of group D were lower than the other group as of the tenth minute of the surgery. There was no significant difference regarding the time to extubation, eye opening an orientation times, Modified Aldrete's Scores, postoperative VAS scores, postoperative nausea and vomiting between the two groups. Conclusion: We have concluded that, use of remifentanyl with desflurane is more suitable compared to the use of remifentanyl with sevoflurane in surgeries when deep hypotension is targeted.

Amaç: Sinüs veya burun cerrahisi geçirecek hastalarda kontrollü hipotansiyon cerrahi sahadaki görüşün iyileştirilmesini ve cerrahi konforun artması açısından önemli iken, hızlı uyanma ve derlenme de hastanın havayolu açıklığının hızla kontrol edebilir hale gelmesi ve cerrahi komplikasyonların takibi açısından önemlidir. Biz de bu çalışmada sevoşuran ve desşuranın perioperatif dönemde hemodinami, postoperatif dönemde derlenme üzerindeki etkileri ve yan etkilerinin karşılaştırmayı amaçladık. Gereç ve Yöntem: Rinoplasti ve/veya septoplasti, fonksiyonel endoskopik sinüs cerrahisi uygulanacak 40 hasta rasgele iki gruba ayrılıp bir gruba sevoşuran (Grup S), diğer gruba desşuran (Grup D) verilirken her iki gruptaki hastalara analjezik olarak remifentanil infüzyonu uygulanarak genel anestezisi uygulandı. Operasyon sonunda hastaların trakeal ekstübasyon, göz açma, emirlere uyma ve oryantasyon süreleri ile derlenme skorları kaydedildi. Bulgular: Gruplar arasında, demografik verileri açısından fark saptanmadı. ‹ntraoperatif dönemde 15, 40, 45, 50, 55, 60, 65. dakikalarda (dk) OAB değerleri, Grup D’de diğer gruba göre düşük bulundu. Operasyonun 10. dakikasından itibaren desşuran grubundaki MAK değerleri, sevoşuran grubuna göre daha düşük bulundu. Ekstübasyon süresi, göz açma ve oryantasyon süresi dağılımları, Modifiye Aldrete Derlenme Skoru, postoperatif VAS değerleri ve postoperatif bulantı-kusma gruplar arasında oranları gruplar arasında anlamlı değildi. Sonuç: Desşuranla remifentanilin birlikte kullanılmasının, remifentanil birlikte kullanılan sevoşurana göre derin hipotansiyon hedeşenen cerrahi işlemlerde daha uygun olabileceği sonucuna vardık.
