Risk Factors in Children with Congenital Anomalies of Kidney and Urinary Tract

Abstract
Objective: Congenital anomalies of kidney and urinary tract (CAKUT) constitute the most important cause of chronic kidney disease in children. There is insufficient data on the risk factors of these anomalies. In this study, it was aimed to investigate risk factors in children with CAKUT. Method: Patients with CAKUT and healthy children as a control group were included in the study. Mothers’ weight gain, use of cigarette, folic acid, iron and a medication during pregnancy, their body mass indexes before pregnancy, gestational week, and birth weight of children were investigated. Results: A total of 266 participants including 142 patients and 124 children without anomalies as a control group were included in this study. The most frequent anomaly was vesicoureteral reflux (n=39), and the second most common one was ureteropelvic junction obstruction (n=30). The frequency of having low birth weight was higher in children with CAKUT than the control group (n=22 [15.5%], n=12 [9.67%], p=0.045, respectively). While 18.3% of the mothers in the patient group but 5.6% of them in the control group were obese, (p=0.006). In logistic regression analysis, a statistically significant relationship was found between obesity and CAKUT (odds ratio: 1.54, confidence interval: 1.122-1.981, p=0.021). The percentage of mothers who gained excess weight during pregnancy was higher in the patient group (17.6% [n=25], 11.3% [n=14], p=0.032, respectively). There was a significant relationship between excess weight gain during pregnancy and the presence of CAKUT (odds ratio: 1.15, confidence interval: 1.075-1.453, p=0.041). Conclusion: Maternal obesity before pregnancy and excess weight gain during pregnancy can be a risk factor for the development of CAKUT. Early investigation of these children during the early stage in terms of CAKUT can be useful in early diagnosis and treatment planning.

Amaç: Doğumsal böbrek ve üriner sistem anomalileri (DBÜSA) çocuklarda kronik böbrek hastalığının en önemli nedenini oluşturmaktadır. Ancak, bu anomalilerin oluşmasındaki risk faktörleri hakkında yeterli veri bulunmamaktadır. Bu çalışmada, DBÜSA olan çocuklarda risk faktörlerinin araştırılması amaçlandı. Yöntem: Çalışmaya DBÜSA olan hastalar ile kontrol grubu olarak anomalisi olmayan çocuklar dahil edildi. Gebelikte annenin kilo alımı, sigara, folik asit, demir ve ilaç kullanımı, gebelikten önceki vücut kitle indeksi, çocukların gebelik haftası ve doğum kilosu araştırıldı. Bulgular: Çalışmaya 142 hasta ve kontrol grubu olarak 124 anomalisi olmayan çocuk dahil edildi. En sık saptanan anomali vezikoüreteral reflü (n=39) olup, ikinci sıklıkta üreteropelvik darlık olduğu (n=30) saptandı. Düşük doğum ağırlığına sahip olma sıklığı DBÜSA olan çocuklarda kontrol grubundan daha yüksekti (sırası ile n=22 [%15,5], n=12 [%9,67], p=0,045). Hasta grubunda annelerin %18,3’ü obez iken, bu oran kontrol grubunda %5,6 olarak bulundu (p=0,006). Lojistik regresyon analizinde obezite ile DBÜSA arasında istatistiksel anlamlı bir ilişki olduğu belirlendi (Odds oranı: 1,54, güvenlik aralığı: 1,122-1,981, p=0,021). Gebelikte fazla kilo alan annelerin oranı hasta grubunda daha fazla idi (sırası ile %17,6 [n=25], %11,3 [n=14], p=0,032). Gebelikte fazla kilo alımı ile DBÜSA varlığı arasında anlamlı bir ilişki mevcuttu (Odds oranı: 1,15, güvenlik aralığı: 1,075-1,453, p=0,041). Sonuç: Gebelik öncesi obezite ve gebelikte fazla kilo alımı doğumsal böbrek ve üriner sistem anomalileri gelişmesi açısından bir risk faktörü olabilir. Bu çocukların erken dönemde araştırılması erken tanı ve tedavinin planlanmasında yararlı olabilir.
