Evaluation of the Central Venous Catheters Access and Malposition Ratios in Cardiac Surgery

Abstract
Objective: The incidence of central venous catheter malposition is 5 -14 % and its incidence is lower for the right internal jugular vein. Our study aims to determine the positions of the introducer and the central venous catheter simultaneously placed into the right internal jugular vein as well as their malposition rates in cardiac surgery. Material and Method: The central venous catheter positioning was retrospectively examined in the patients who underwent a cardiac operation between 2013 and 2016. The ideal place for the tip of the catheter was considered as two zones over the carina: Zone A, opening of the right atrium and lower superior vena cava (SVC) and Zone B, upper SVC and junction of the right innominate vein. The tip of catheter positioned below the carina was evaluated as Zone C (in the right atrium), whereas the positioning of a catheter outside the superior vena cava was considered to be a malposition.Results: Of 392 patients, 133 had the catheter tip in Zone A, 121 in Zone B, and 99 in Zone C, whereas 39 (9.94%) catheters were malpositioned (Zone D). Most frequent venous malposition was observed in vena cava inferior. When the patients were divided into two groups as 165 cm (tall, n=202) by their height, lesser number of patients had catheters placed in Zone A (the ideal place), while higher number of malpositioned catheter was detected (27 vs 12) in short patients.Conclusion: We haven’t any study encountering the impact of implantation of double catheter on malposition and ours is the first study conducted on this issue. We conclude that the rate of catheter tip malposition is higher in short patients when two catheters were implanted in the same right internal jugular vein

Amaç: Santral venöz kateter malpozisyon oranı % 5 -14 olup, sağ internal juguler ven için daha azdır. Çalışmamızın amacı, sağ internal juguler vene eşzamanlı olarak konulan introducer ve santral venöz kateterin yerleşim yeri ve malpozisyon oranlarını belirlemektir.Gereç ve Yöntem: 2013-2016 yılları arasında kardiyak cerrahi yapılan hastalarda santral venöz kateter yerleşimi retrospektif olarak incelendi. Kateter ucunun yeri karinanın üstünde iki bölgeye ayırıldı: Zon A, sağ atrium atrium girişi ve alt vena kava süperior ve Zon B, üst vena kava süperior ile sağ innominat ven kavşağı. Karinanın altında yerleşen kateter ucu Zon C (sağ atrium içi) olarak değerlendirildi. Vena kava süperior dışında venöz sistemde kateter yerleşimi ise malpozisyon (Zon D) olarak kabul edildi.Bulgular: Üç yüz doksan iki hastadan 133'ü Zon A, 121'i Zon B ve 99'u Zon C iken malpozisyon sayısı (Zon D) 39 (% 9,94) idi. En sık izlenen venöz malpozisyon yeri vena kava inferior idi. Hastalar boylarına göre, 165 cm altında (Grup Kısa, n=190) ve üstünde (Grup Uzun, n=202) olarak 2 gruba ayrıldığında, kısa boylu hastalarda Zon A (ideal yer) olan hasta sayısı az iken, malpozisyon daha fazla idi (27 vs 12). Sonuç: Çift kateter takılmasının malpozisyon üzerine etkisini gösteren bir literatüre rastlanılmamış olup çalışmamız bu alanda yapılan ilk çalışmadır. Özellikle kısa boylu hastalarda aynı vene çift santral venöz kateter tekniği ile kateter ucunun yanlış yerleşim oranının daha fazla olduğu sonucuna varıldı
