Efficacy of polyamide net coated with silicone in burn patients

Abstract
Aim: Polyamide net coated with silicone (PAS) is a material with no additive biologic compounds and it adheres to healthy tissue but not to burned tissue. As a consequence, it does not disturb new epithelium formation and enhances tissue healing. Deuto of its multipore structure, it provides passage of antibiotics to the injured tissue while facilitates clearance of burned area from hematoma and exudates. In this study, the results of PAS and povidone-iodine dressings in burn treatment were compared. Methods: 54 patients, admitted between February 1998 and October 2002, were analysed retrospectively in regards to age, sex, mechanism of burn, percentage of burned area, presence of infection, eschar formation, operative intervention, and duration of hospital stay. The patients treated with debridement and povidone-iodine were classified as group I, and the ones treated with PAS (Mepitel®) dressing were classified as group II. Results: There were 27 patients in each group. In group I, mean age and male to female ratio was 27 months and 1.2/1, respectively, whereas it was 32 months and 2/1 in group II. Majority of the burns were scalds and each group was included one patient burned with flame burn. Mean percentage of burned area was 12.1 in both groups and all burns were recorded as deep second degree. There were no infections in group II, nevertheless one patient had Pseudomonas aeruginosa and the another had Staphylococcus epidermidis infections in group I. Percentage of eschar formation in group I and II were % 55 and % 22, respectively. Although 5 patients in group I required debridment under general anaesthesia, only 1 patient in group II underwent similar surgery. Length of hospital stay for group I and II were 13 and 5 days, respectively. Conclusion: Treatment with PAS dressing of childhood burns seems superior to debridement and povidone-iodine dressing with its advantages of shortening the duration of hospital stay, decreasing eschar formation and the need for grafting.

Amaç: Silikon ile kaplanmış polyamide ağ (SPA) sağlıklı dokuya yapışan ancak yara dokusuna yapışmadığı için yeni epitel oluşumunu bozmayan ve başka biyolojik bileşikler içermeyen bir pansuman malzemesidir. Multipor özelliğinden dolayı yara dokusuna antibiyotik geçişine izin verirken, hematom ve eksudanın yarayı terk etmesini sağlar. Bu çalışmada yanık tedavisinde SPA ve povidon iyodin uygulamalarının sonuçları karşılaştırılmıştır. Yöntem: Şubat 1998 ile Ekim 2002 tarihleri arasında izlenmiş olan 54 hasta geriye dönük olarak yaş, cinsiyet, yanık oluş mekanizması, yanık alanı yüzdesi, enfeksiyon varlığı, eskar oluşumu, operasyon gereksinimi ve hastanede kalış süreleri açısından incelendi. Klasik debridman ve povidon iyodin uygulanan hastalar grup I, SPA (Mepitel®) uygulanan hastalar grup II olarak sınıflandırıldı. Bulgular: Her grupta 27 hasta vardı. Grup I'de yaş ortalaması 27 ay, erkek/kız:1.2/1; grup II de 32 ay, erkek/kız: 2/1 idi. Yanıkların oluş mekanizması çoğunlukla sıcak su ile haşlanma iken her iki grupta da birer alev yanığı vardı. Ortalama yanık yüzdesi her iki grupta da % 12.1 idi. Tüm yanıklar derin ikinci dereceydi. Grup II de enfeksiyon saptanmazken, grup I'de bir hastada Pseudomonas aeruginosa, bir diğer hastada Staphylococcus epidermidis üredi. Eskar formasyonu grup I/grup II: % 55/% 22. Grup I'de 5 hastada genel anestezi altında debridman gerekli olurken, SPA grubunda yalnızca bir hastada debridman yapıldı. Hastanede kalış süresi grup I'de ortalama 13 gün, grup II'de ise 5 gün idi. Sonuç: Çocuk yanıklarında SPA uygulaması hastanede kalış süresini kısaltması, eskar oluşumunun ve greft gereksiniminin daha az olması nedeniyle debridman ve povidon iyodin uygulamasına oranla daha etkili görünmektedir.
