Psychiatric assessment in children and adolescents treated at the child surgery inpatient clinic

Abstract

Objective: Some psychiatric problems are seen after surgical operation in children and adolescents. Prognosis and adjustment of treatment can be negatively affected by these psychiatric problems. In this study, general psychiatric symptoms and behavioral conditions have been assessed in patients after the surgical operation in the child surgery inpatient clinic. Method: All of subjects have been assessed at the department of child surgery of Istanbul medical faculty. 101 (63 male, 38 female) subjects were involved in this study. Subjects and parents have been evaluated by CBCL (Child Behavior Check List) and socio-demographic form. Results: Mean age was 8.42±3.53 years according to our results. There is not any significant statistical differentiation between male and female subjects according to their CBCL scores (p>0.05). The highest scores were obtained from the children and adolescent who were operated due to acute appendicitis and corosive injuries of the esophagus. The mean T scores on the behavior problems, internalizing, externalizing, withdrawn, aggression, anxious-depressed, attention problems, social problems, in the study group were significantly higher than the scores in the community group (p<0.05). Conclusion: Surgical operations can cause some behavioral and psychiatric problems in the children and adolescents who are operated in the child surgery units. Especially, some surgical operations are more important for psychiatric problems. Therefore Child surgery inpatient clinics are often needed for cooperation with consultation liaison psychiatry workers. This cooperation is very important for children and adolescents with psychiatric problems due to good prognosis and adjustment of the treatment.

Amaç: Çocuk ve ergenlerde cerrahi müdahale sonrası bazı psikiyatrik problemler oluşabilmektedir. Bu problemler yatan hastanın tedaviye uyumunu ve hastalığın prognozunu olumsuz olarak etkileyebilmektedir. Bu çalışmada çocuk cerrahisi servisinde, cerrahi müdahale sonrası yatan hastaların genel davranışsal durumları ve psikiyatrik semptomları değerlendirilmiştir. Metod: İstanbul Tıp Fakültesi, Çocuk Cerrahisi Yatan Hasta Servisi'ndeki çocuk ve ergenler değerlendirmeye alınmıştır. Çalışmaya 101 (63 erkek, 38 kız) çocuk ve ergen alınmıştır. Çocuklar ve aileleri sosyodemografik veri formu ve CBCL (Child Behavior Check List) ile değerlendirilmiştir. Sonuçlar: Elde edilen bulgulara göre yaş ortalaması 8.42±3.53 olarak bulundu. CBCL alt ölçek puanları açısından cinsiyetler arası fark bulunmadı (p>0.05). En yüksek puanlar akut apandisit ve korozif osefajit operasyonu geçiren çocuk ve gençlerde tespit edildi. Bütün çocukların agresyon, içe yönelim, dışa yönelim, dikkat problemleri, anksiyete ve depresyon, sosyal problemler, alt ölçek puanları kontrol grubuna göre anlamlı derecede yüksek bulundu (p<0.05). Tartışma: Çocuk cerrahisi ünitesinde operasyon geçiren çocuk ve ergenlerde, cerrahi operasyonlar bazı davranışsal ve psikiyatrik problemler oluşturabilir. Özellikle, bazı operasyonlar psikopatoloji açısından önemli olmaktadır. Bu nedenle, çocuk cerrahisi servislerinde konsültasyon liyezon psikiyatrisi çalışanlarının işbirliği gerekmektedir. Bu işbirliği yüksek psikopatoloji gösteren çocukların iyi prognozu ve tedaviye uyumu açısından önemlidir.
