SURGICAL RESULTS OF CEREBELLOPONTINE ANGLE EPIDERMOID TUMORS

Abstract
Introduction:Epidermoid tumors represent 0.3% to 1.8% of all primary intracranial tumors. They are the third most common cerebellopontine angle tumor, comprising approximately 7% of cerebellopontine angle pathology. Material and Method:Between 2010 and 2020, 11 patients who were operated with cerebellopontine angle tumors and given a diagnosis of epidermoid tumors were analyzed retrospectively. Patients were evaluated in terms of demographic characteristics, presenting symtoms, the location and size of the tumor, postoperative complications and recurrence. Results:A total of 11 patients were diagnosed with epidermoid tumors. The average age of the patients was 48.2. 8 of the patients were male and 3 were female. The most common presenting symtoms were headache, dizziness, imbalance, hearing loss and loss of face sensation. 7 patients had tumor located in the right cerebellopontine angle, and 4 patients in the left angle. All patients underwent surgery and tumor excision was performed in the lateral decubitis position with a suboccipital retrosigmoid approach using a microsurgical technique. The most common postoperative complications were diplopia, lateral gaze limitation and transient worsening of facial nerve. Mollaret’s meningitis developed in two patients. Recurrence was observed in two patients during the postoperative follow-up. Conclusion:Epidermoid tumors, one of the three most common tumor types in this region, should be considered in the differential diagnosis of the cerebellopontine angle tumors. In the surgical resection of epidermoid tumors, capsule removal may not always be possible, leading to morbidity and mortality. Patients should be followed closely in terms of Mollaret’s meningitis during postoperative follow-up.

Giriş: Epidermoid tümörler tüm primer beyin tümörlerinin %0,3 ile %1,8’ini oluştururlar. Serebellopontin köşede yerleşen tümörlerin %7’sini oluşturur ve serebellopontin köşede en sık görülen üçüncü tümördür. Gereç ve Yöntem: 2010-2020 yılları arasında kliniğimizde serebellopontin köşe tümörü tanısıyla opere edilen ve patoloji sonucu epidermoid tümör saptanan 11 hasta retrospektif olarak incelendi. Hastalar demografik özellikler, başvuru şikayetleri, kitlenin yerleşim yeri, boyutu, postoperatif komplikasyonlar ve nüks açısından değerlendirildi. Bulgular: Toplam11 hastaya epidermoid tümör tanısı konuldu. Hastaların 8’i erkek, 3’ü kadın, ortalama yaşları 48.2 idi. En sık başvuru şikayetleri, başağrısı, baş dönmesi, dengesizlik, işitme kaybı ve yüzde his kaybıydı. 7 hastada sağ, 4 hastada sol serebellopontin köşe yerleşimli tümör mevcuttu. Tüm hastalar cerrahiye alınarak lateral dekübit pozisyonunda suboksipital retrosigmoid yaklaşımla mikroşirürjikal teknikle tümör eksizyonu uygulandı. Postop en sık komplikasyon olarak çift görme, lateral bakış kısıtlılığı ve fasial sinir fonksiyonunda geçici gerileme görüldü. İki hastada Mollaret menenjiti gelişti. Postoperatif dönemde takiplerde iki hastada nüks görüldü. Sonuç: Serebellopontin köşe yerleşimli tümörlerin ayırıcı tanısında bu bölgede en sık üç tümör tipinden biri olan epidermoid tümörler akla gelmelidir. Epidermoid tümörlerin cerrahi rezeksiyonunda kapsül çıkarılması her zaman mümkün olmayabilir, morbidite ve mortaliteye yol açabilir. Postoperatif takipte Mollaret menenjiti açısından hastalar yakın takip edilmelidir.
