Viral and Parasitic Gastroenteritis Agents and Metronidazole Treatment in Diyarbakir Children’s Hospital

Abstract

Introduction: Acute gastroenteritis (AGE) agents vary according to the hygiene and sanitation conditions of the regions and age groups of the patients. Rotavirus is the most commonly reported agent in all ages, whereas adenovirus is usually detected in sporadic cases. Giardia lamblia is a common gastroenteritis protozoa worldwide. The incidence of giardiasis approaches 40% in regions with insufficient sanitation and hygiene conditions. Amibic colitis is a parasitic infection caused by Entamoeba histolytica. Metronidazole is a synthetic 5-nitroimidazole affecting anaerobic bacteria and protozoa. Unnecessary usage of metronidazole, which is widely prescribed in the treatment of amebiasis and giardiasis, may lead to antibiotic resistance as well as intestinal dysbiosis. In this study, we investigated rotavirus, adenovirus, G. lamblia and E. histolytica in the stool samples and metronidazole presciption of AGE patients admitted to our hospital. Materials and Methods: The fecal samples of 8874 patients (6 months-16 years) admitted to Diyarbakir Pediatrics Hospital with the diagnosis of AGE were retrospectively analyzed. Rotavirus, adenovirus, G. lamblia and E. histolytica/dispar agents were investigated by Enzyme Immun Assay (EIA) method using RIDASCREEN® (R-biopharm AG, Germany) commercial kits. Fresh stool samples were also evaluated for G. lamblia and Entamoeba cyst and trophozoites by native-lugol (NL) method. Results: Among the 8874 samples, G. lamblia (1730 samples, 19.5%) was the most common agent followed by rotavirus (1083 samples, 12.2%). Adenovirus and E. histolytica/dispar were detected in 276 (3.1%) and 189 (2.2%) samples, respectively. Among the 3162 patients who were prescribed with metronidazole, G. lamblia, rotavirus, E. histolytica/dispar and adenovirus was found in 1730, 124, 194 and 35 patients, respectively. Metronidazole was prescribed in 1238 (17.8%) of the 6950 patients who were not diagnosed as giardiasis or amoebiasis by laboratory methods. Conclusion: Metronidazole prescription rate without laboratory findings of giardiasis or amoebiasis was high in our hospital. The proper use of laboratory methods in the diagnosis of acute gastroenteritis and the regulation of treatment after laboratory-based diagnosis will reduce unnecessary antibiotics usage.

Giriş: Akut gastroenterit (AGE) etkenleri, bölgelerin hijyen ve sanitasyon koşullarına ve hastaların yaş gruplarına göre değişkenlik gösterir. Rotavirüs her yaş grubunda en sık bildirilen etkendir, adenovirüs ise genellikle sporadik olgularda saptanır. Giardia lamblia dünyada yaygın olarak görülen, gastroenterit etkeni bir protozoondur. Giardiyaz insidansı çevre sanitasyon koşullarının yetersiz olduğu bölgelerde %40’lara yaklaşır. Amibik kolit, Entamoeba histolytica’nın neden olduğu, nadir görülen ancak tanısında sorunlar yaşanan bir parazit infeksiyonudur. Metronidazol, anaerop bakteri ve protozoonlara etkili sentetik 5-nitroimidazol türevidir. Amebiyaz ve giardiyaz tedavisinde yaygın olarak reçete edilen metronidazolün gereksiz kullanımı, intestinal disbiyozun yanı sıra antibiyotik direncine de neden olabilir. Bu çalışmada, AGE tanısıyla hastanemize başvuran hastaların dışkı örneklerinde rotavirüs, adenovirüs, G. lamblia ve E. histolytica varlığı ve hastalara metronidazol reçete edilme durumu araştırılmıştır. Materyal ve Metod: Diyarbakır Çocuk Hastalıkları Hastanesi klinik ve polikliniklerinden AGE ön tanısıyla Mikrobiyoloji Laboratuvarına gönderilen 6 ay-16 yaş arasındaki 8874 hastaya ait dışkı örnekleri retrospektif olarak incelendi. Rotavirüs, adenovirüs, G. lamblia ve E. histolytica/dispar etkenlerinin varlığı RIDASCREEN® (R-biopharm AG, Almanya) ticari kitleri kullanılarak enzim immünassay (EIA) yöntemiyle araştırıldı. Taze dışkı örnekleri nativ-lugol (NL) yöntemiyle G. lamblia ve E. histolytica kist ve trofozoit varlığı açısından da değerlendirildi. Bulgular: İncelenen 8874 dışkı örneğinde en sık saptanan etken G. lamblia (1730 örnek, %19.5) olup, onu rotavirüs (1083 örnek, %12.2) izlemiştir. Örneklerin 276 (%3.1)’sında adenovirüs, 189 (%2.2)’unda ise E. histolytica/dispar saptanmıştır. Metronidazol reçete edilen toplam 3162 hastanın 1730’unda G. lamblia, 124’ünde rotavirüs, 194’ünde E. histolytica/dispar, 35’inde adenovirüs saptanırken 1079 hastada bu etkenlerin hiçbiri saptanmamıştır. Dışkı örneğinde G. lamblia veya E. histolytica/dispar saptanmayan 6950 hastanın 1238 (%17.8)’ine metronidazol reçete edilmiştir. Sonuç: Mikroskobik inceleme ve yüksek duyarlılıkta antijen testi uygulanan bir merkezde giardiyaz veya amibik kolit laboratuvar bulgusu olmadan metronidazol reçete edilme oranı yüksektir. Akut gastroenterit tanısında laboratuvar yöntemlerinin doğru ve yerinde kullanılması ve tedavinin laboratuvar destekli tanı sonrasında düzenlenmesi gereksiz antibiyotik kullanımını azaltacaktır.
