A polysomnographic evaluation of patients with nocturnal seizures

Abstract

Objectives: We investigated the contribution of polysomnography (PSG) to daytime and routine EEG findings with regard to observation of seizures, detection of epileptic foci, and documentation of sleep stages at which culmination of epileptic activity occurs. Patients and Methods: The study included 22 patients (9 men, 13 women; mean age 38 years; range 17 to 55 years) with nocturnal seizures. Diagnoses of seizure types according to patient histories were primary generalized in 15 patients (68%) and complex partial in seven patients (32%). The patients were hospitalized for a night to perform 8-hour-PSG recordings. The results were compared with routine and daytime long-term EEG findings for various seizure types. Results: During PSG, seizures were observed in two patients while epileptic foci were documented in 14 patients (64%). Of these, epileptic foci were on the left in 10 patients (71.4%), on the right in two patients (14.3%), and bilateral in two patients (14.3%). Compared with PSG recordings, repeated routine EEGs and daytime long-term EEGs revealed foci in 11 patients (50%) and 10 patients (45.5%), respectively. Conclusion: Observation of seizures and documentation of epileptic foci by means of PSG may prove useful in the diagnosis, follow-up, and prognosis of patients with pure nocturnal seizures, and in the selection of appropriate antiepileptic drugs.

Nokturnal nöbetli hastalarda, polisomnografi (PSG) incelemesi ile nöbeti gözlemek, nöbet tipini, epileptik odağı ve epileptik aktivitelerin uykunun hangi döneminde yoğunluk kazandığını belirlemek ve PSG'nin rutin ve gündüz uzun süreli EEG incelemelerine katkısını saptamak. Hastalar ve Yöntemler: Çalışmaya pür nokturnal nöbet nedeniyle izlenen 22 hasta (9 erkek, 13 kadın; ort. yaş 38; dağılım 17-55) alındı. Klinik öykü bilgilerine göre, 15 hasta (%68) primer jeneralize nöbet, yedi hasta (%32) kompleks parsiyel nöbet öntanısıyla izlenmekteydi. Hastalar bir gece hastaneye yatırılarak sekiz saat süreyle PSG ile izlendi. Sonuçlar, olguların anamnez özelliklerine göre belirlenen nöbet tipleri ile rutin ve gündüz uzun süreli EEG patolojileri ile karşılaştırıldı. Bulgular: Polisomnografi incelemesi sırasında iki hastada klinik nöbet gözlendi; 14 hastada (%64) epileptik odak saptandı. Epileptik odak saptanan hastaların 10'unda (%71.4) sol hemisferde, ikisin-de (%14.3) sağ hemisferde, ikisinde (%14.3) ise iki hemisferde iki ayrı odak belirlendi. Tekrarlayan rutin EEG'lerle 11 hastada (%50), gündüz uzun süreli tek EEG ile 10 hastada (%45.5), PSG ile ise 14 hastada (%64) patoloji saptandı. Sonuç: Polisomnografinin pür nokturnal nöbetli olgularda, nöbet gözlemi ve epileptik odak saptanmasındaki rolü nedeniyle tanı, izlem, ilaç seçimi ve prognoz açısından katkı sağlayabileceği düşünüldü.
