Evaluation of the prognostic classifications in esophageal atresia and tracheoesophageal fistula

Abstract
Aim: The purpose of this analysis was to investigate outcomes in newborns with esophageal atresia (EA) and/or tracheoesophageal fistula (TEF) with respect to prognostic classifications and complications in our department. Material and Method: Charts of all 76 infants with EA/TEF treated at Akdeniz University Hospital, Department of Pediatric Surgery from 1989 to May 2005 were reviewed. Patient demographics, accompanying anomalies, complications and survival rates were examined. The results obtained were compared with available prognostic classifications. Results: Applying Waterston's prognostic classification to our patient population, survival rates were 100 % for class A, 70.8 % for class B, 40 % for class C. According to the Montreal classification, survival rates were 74.6 % for class 1, 23.5 % for class 2. Using the Spitz's classification, survival rates were 62.7 % for class 1, 70 % for class 2. When the Bremen's classification were applied, survival rates were 90 % for class 1, 100 % for class 2 "without complications", 23.8 % for class 1, 40 % for class 2 "with complications". Complications included preoperative pneumonia and respiratory distress. Conclusions: Comparing the major prognostic classifications, the Waterston's and Bremen's classification schemes were found to be most applicable. However, Montreal's and Spitz's classifications failed to include survival related to sepsis and other complications

Amaç: Bu çalışmanın amacı, kliniğimizdeki özofagus atrezisi (ÖA) ve/veya trakeoözofageal fistül (TÖF) bulunan hastaların prognostik sınıflamasını ve komplikasyonları ile ilgili sonuçları incelemektir. Gereç ve Yöntem: ÖA/TÖF nedeniyle 1989 ile Mayıs 2005 yılları arasında Akdeniz Üniversitesi Hastanesi Çocuk Cerrahisi Anabilim Dalında tedavi edilen 76 bebeğin bilgileri gözden geçirildi. Hastalara ait demografik veriler, eşlik eden anomaliler, komplikasyonlar ve yaşam oranları incelendi. Elde edilen sonuçlar mevcut prognostik sınıflamalarla karşılaştırıldı. Bulgular: Waterston prognostik sınıflaması uygulandığında yaşam oranları, A grubunda % 100; B grubunda % 70.8; C grubunda % 40 idi. Montreal sınıflamasına göre yaşam oranları ise, 1. grupta % 74.6; 2. grupta % 23.5 idi. Spitz sınıflaması kullanıldığında yaşam oranları, 1. grupta % 62.7; 2. grupta % 70 idi. Bremen sınıflamasına göre sağkalım oranları incelendiğinde komplikasyon olmayan 1. grupta % 90; 2. grupta % 100 iken, komplikasyon olan 1. grupta % 23.8; 2. grupta % 40 idi. Komplikasyonlar ameliyat öncesindeki pnömoni ve respiratuar distresi içermekteydi. Sonuç: Prognostik sınıflandırmalar karşılaştırıldığında Waterston ve Bremen sınıflamaları serimize daha uygulanabilir olarak bulunmuştur. Montreal ve Spitz sınıflamaları ise sepsis ve buna benzer komplikasyonları içermemesi nedeniyle yaşam oranlarını değerlendirmede yetersiz kalmaktadır.
