Percutaneous endoscopic gastrostomy: Our experiences

Abstract
Aim: Percutaneous endoscopic gastrostomy (PEG) was first developed for children as a minimally invasive method. After it was described, first adult gastroenterologists started to perform PEG, followed by general surgeons and pediatric gastroenterologists, as pediatric surgeons being the last group to accept this technique. Early and late complications may occur during PEG procedure. However these complications can be prevented by strictly obeying the technical rules and a careful follow up after the procedure. Herein we summarized the clinical features of our patients who had PEG. Material and Method: The hospital records of 8 patients who underwent PEG during 2000- 2004 were evaluated retrospectively. In all patients, PEG procedure was performed as Gauderer had described. Results: Between 2000- 2004, PEG procedure was performed to 8 patients aging 1-13 years (avarage 5,5). Among them, fundoplication was performed to 3 patients with symptomatic gastroesophageal reflux (GER). In the other 5 patients, GER symptoms did not occur after PEG procedure. Durig their follow up, one PEG catheter was taken out from a patient after postoperative 19 months for sufficient oral feeding and one catheter was replaced by a button after 6th months because of catheter breakage. Conclusion: PEG procedure is an altenative method to open gastrostomy which increases the life quality of patients when performed with a correct technique.

Amaç: Perkütan endoskopik gastrostomi (PEG) çocuklar için "az invaziv" bir yöntem olarak geliştirilmiştir. PEG tanımlandıktan sonra ilk olarak erişkin gastroenterologların ve genel cerrahların, ardından çocuk gastroenterologlarının, en son çocuk cerrahlarının yöntemi kullandığı saptanmıştır. PEG uygulaması sırasında, erken dönemde ve geç dönemde komplikasyonlar görülebilmektedir. Komplikasyonlar uygulama sırasında tekniğe dikkat edilmesi ve uygulama sonrasında sık takip ve bakımın iyi yapılması ile en aza indirilebilir. Çalışmamızda PEG uyguladığımız hastaların özellikleri irdelendi. Gereç ve Yöntem: Kliniğimizde 2000-2004 yılları arasında 8 çocuk hastaya kıvrılabilir endoskop kullanılarak PEG kateteri uygulandı. Tüm hastalarda PEG tekniği olarak Gauderer'in tarif ettiği klasik teknik kullanıldı. Hastaların dosyaları geriye dönük olarak değerlendirildi. Bulgular: Yaşları 1-13 arasında (ortalama 5,5) olan 8 hastaya PEG takıldı. Gastroözefajiyal reflü (GÖR) semptomları olan ve reflü sintigrafisi ile tanı konulan 3 hastaya PEG öncesinde antireflü ameliyatı yapıldı. GÖR saptanmayan 5 hastada, PEG uygulanması sonrasında GÖR semptomu gelişmedi. Bir hastada kateter takıldıktan 19 ay sonra ağızdan beslenmesi yeterli olduğu için çıkartıldı. Sadece bir hastada kateter takıldıktan 6 ay sonra kateter yırtıldı. Yırtılan kateter çıkartılarak yerine "button" tabir edilen düğme takıldı. Sonuç: PEG kateteri uygun teknik ile yerleştirildiğinde hastaların yaşam kalitesini artıran açık gastrostomiye alternatif bir yöntemdir.
