Intravenous Lipid Emulsion Treatment After High Dose Propafenone Intake for Suicidal Ideation

Abstract
Propafenone is a class 1C antiarrhythmic drug. It acts mainly by blocking calcium channels. Propafenone is a lipophilic drug and more than 90% of the dru gis bound to plasma proteins. Its IV lipid emulsion formulation forms a separate lipid compartment in the plasma, keeps lipophilic drugs in this compartment and reduces the unwanted effects of these drugs. We aimed to demonstrate the efficacy of IV lipid emulsion treatment we used in addition to standard treatments in this patient who consumed high doses of propafenon for suicidal attempt , and developed cardiac complication.
Özet
Propafenon sınıf 1C antiaritmik bir ilaçtır. Daha çok kalsiyum kanal blokajı yaparak etki gösterir. Lipofilik bir ilaç olan propafenon, plazma proteinlerine %90’dan fazla oranda bağlanır. IV lipid emülsiyonu, plazmada ayrı bir lipid kompartmanı oluşturarak lipofilik ilaçları burada tutmakta ve bu ilaçların istenmeyen etkilerini azaltmaktadır. Bu olgumuzda intihar amaçlı yüksek doz propafenon alan ve kardiyak komplikasyon gelişen bir olguya standart tedavilere ek olarak uyguladığımız IV lipid emülsiyonu tedavisinin etkinliğini vurgulamak istedik.
