Evaluation of Bacterial Factors Determined By Polymerase Chain Reaction in Lower Respiratory Tract Infections

Abstract
Introduction: In this study, we aimed to determine the frequency of bacterial agents by polymerase chain reaction (PCR) method obtained from nasopharyngeal swab specimens a tertiary care hospital. Materials and Methods: Between January 2017 and December 2017, 352 nasopharyngeal swab specimens sent to the microbiology laboratory for detection of bacterial agents by PCR from various units of our hospital were retrospectively reviewed. Results: A total of 352 cases were evaluated. At least one bacterial agent was detected with PCR in 58 (16%) of the 352 cases; 275 (78%) had no bacterial agent. In 19 (5%) of the cases more than one factor was detected. The most common bacterial agents were Streptococcus pneumoniae in 23 (7%), Haemophilus influenzae in 17 (5%), Bordetella pertussis in 6 (2%), Bordetella parapertussis in 6 (2%), Chlamydophila pneumoniae in 3 (1%), Mycoplasma pneumoniae in 2 (1%), and Legionella pneumophila in 1 (0.3%). In dual agent infections, 16 (5%) were S. pneumoniae and H. influenzae, 2 (1%) were B. pertussis and C. pneumoniae and 1 (0.3%) was M. pneumoniae and C. pneumoniae. Conclusion: Detection of bacterial factors by PCR in lower respiratory tract infections will prevent unnecessary antibiotic use by providing early diagnosis.

Giriş: Bu çalışmanın amacı, üçüncü basamak bir hastanede nazofarengeal sürüntü örneklerinde polimeraz zincir reaksiyonu (PCR) yöntemiyle bakteriyel etkenlerin sıklığının belirlenmesidir. Materyal ve Metod: Ocak 2017-Aralık 2017 tarihleri arasında hastanemizin çeşitli birimlerinden PCR ile bakteriyel etkenlerin saptanması amacıyla mikrobiyoloji laboratuvarına gönderilen 352 nazofarengeal sürüntü örneği geriye dönük olarak incelendi. Bulgular: Toplam 352 olgunun sonucu değerlendirilmiştir. Değerlendirilen 352 olgunun 58 (%16)’inde PCR ile en az bir bakteri saptanırken, 275 (%78)’inde bakteriyel etken saptanamamıştır. Olguların 19 (%5)’unda birden fazla etken tespit edilmiştir. En sık bulunan bakteriyel etkenlerin sırasıyla 23 (%7)’ü Streptococcus pneumoniae, 17 (%5)’si Haemophilus influenzae, 6 (%2)’sı Bordetella pertussis, 6 (%2)’sı Bordetella parapertussis, 3 (%1)’ü Chlamydophila pneumoniae, 2 (%1)’si Mycoplasma pneumoniae, 1 (%0.3)’i Legionella pneumophila idi. İkili etkenlerin ise 16 (%5)’sı S. pneumoniae ve H. influenzae, 2 (%1)’si B. pertussis ve C. pneumoniae, 1 (%0.3)’i M. pneumoniae ve C. pneumoniae olarak saptanmıştır. Sonuç: Alt solunum yolu infeksiyonlarında PCR ile bakteriyel etken saptanması erken tanı sağlayarak gereksiz antibiyotik kullanımını önleyecektir
