Etiology of fever accociated upper respiratory infections in day-care children

Abstract

Aim: Although most of the acute upper respiratory tract infections are caused by viruses or atypic bacteria which are not treated by antibiotics and heal spontaneously, unnecesseryly physicians usually prescribe antibiotics for the treatment of them. Mothers tend to be anxious about especially fever and impress on physicians for prescribing antibiotics. Children in the day-care centers were the ones who are mostly being prescribing antibiotics for the acute upper respiratory tract infections. So, we aimed to show the viruses as the cause of the most of the acute upper respiratory tract infections and educate the parents. Material and Method: Twenty five patients from a local day-care center who were admitted with fever and diagnosed as upper respiratory tract infection at our outpatient clinic between October 2004-January 2005 were enrolled in this study. All patients were evaluated by the same physician and a form which consist the presence of immunization status of influenza was filled for each case. The patients were investigated for group A beta hemolytic streptococcus in nasopharyngeal swab and influenza type A and B, parainfluenza, RSV and adenoviruses in nasal swab. Findings: The mean age of the patients was 3.3±1.5 (1-6 years) years. Twenty five patients ( 9 male, 16 female) presented with fever ( ? 38°) were diagnosed as upper respiratory tract infection. Influenza type A from 7 patients (28 %), RSV from 7 patients (28 %) and Group A beta hemolytic streptococcus from 1 patient (4 %) were isolated, respectively. Influenza type A was isolated from 36.8% of the patients who were not immunized against influenza but couldn’t be isolated from the nasal swab of the patients who were immunized against influenza. Conclusion: The primary care physicians and the families should be informed about the vaccination of the children against influenza in the day-care centers in respect to not prescribe antibiotics for each patient presenting with acute upper respiratory tract infections with fever except GAS tonsillitis.

Özet

Akut üst solunum yolu infeksiyonları (ÜSYİ) çoğunluğu viral veya antibakteriyel tedavi gerektirmeyen bakterilerle oluşmasına ve kendiliğinden iyileşmesine karşın yanlış olarak çok sık antibiyotik yazılma nedenidir. Özellikle ateş anneleri huzursuz eden ve doktorların antibiyotik yazma konusunda baskı altına alınmalarına neden olan bir semptomdur. Amaç: Akut ÜSYİ nedeni ile en sık antibakteriyel ilaç kullanılan grubu oluşturan yuva çocuklarında, ateşli ÜSYİ’de (tonsillit, tonsillofarenjit, nazofarenjit) etiyolojik araştırma yapılarak antibakteriyel tedavi gerektirmeyen virusların çoğunlukla etken olduğunu göstererek ailelerin eğitilmesi amaçlandı. Gereç ve Yöntem: Ekim 2004-Ocak 2005 tarihleri arasında genel pediatri polikliniğinde ateşli ÜSYİ tanısı konan Sevda Sabancı Kreş ve Anaokulu çocukları ile çalışma yürütüldü. Vakalar tek bir pediatri uzmanı tarafından değerlendirildi ve influenza aşılanma durumunun da kaydedildiği vaka izlem formu dolduruldu. Vakaların nazofaringeal sürüntü kültürlerinde grup A beta hemolitik streptokok (GAS) ve nazal kültürlerinde influenza A, B, parainfluenza, Respiratuvar Sinsisyal Virus (RSV) ve adenovirus araştırıldı. Bulgular: Dört aylık dönemde izlenen çocukların yaş ortalaması 3.3±1.5 yıl (1-6 yaş)’dı. 16’sı (% 64) kız, 9’u (% 36) erkek 25 çocuk 38°C ve üzerinde ateşle birlikte ÜSYİ tanısı aldı. Etken olarak vakaların 7’sinde (% 28) influenza A, 7’sinde (% 28) RSV ve nazofarengeal sürüntüden yalnızca bir vakada (% 4) GAS izole edildi. İnfluenza A vakalarının tümü Aralık-Ocak aylarında görüldü. Aşısızların % 36.8’inde influenza A izole edilirken, aşılıların hiçbirinde influenza A saptanmadı. Sonuç: Ateşli ÜSYİ’li çocukları antibiyotik ile tedavi etmek yerine özellikle yuva çocuklarını influenza aşılaması ile korumak ve yalnızca GAS pozitif vakaları antibiyotikle tedavi etmek ailelere ve birinci basamak hekimlerine benimsetilmelidir.
