Methanol Intoxication in Patient due to the Chronic Usage of Alcohol and Cologne

Abstract

Methyl alcohol is toxic liquid that is found in substances such as mercury and antifreeze. Methanol intoxication is severe disease and may cause death. Sixty years old man was admitted to our emergency clinic complaints of consciousness. İn his history he is chronic alcoholic and usage of cologne. He was transferred to intensive care unit and sedatisized and curarized due to the his glasgow coma scale under eight. 40% ethanol, folic acid, thiamine and 5% dextrose were given with nazogastric way. Sodium bicarbonate was given due to the acidosis. The patient was discharged with complaintsts of blindness after ninth day of his admission

Metil alkol zehirli bir sıvı olup boya, cila, antifriz gibi maddelerin yapısında bulunur. Metanol zehirlenmesi şiddetli hastalık ve ölüme sebep olabilen klinik bir durumdur. Altmış yaşında erkek hasta acil servisimize bilinç bulanıklığı, genel durum bozukluğu sebebiyle getirildi. Anamnezinde kronik alkol aldığı ve zaman zaman kolonya içtiği öğrenildi. Acil servise başvurmadan 3 saat önce her zaman aldığı yer dışında bir yerden kolonya alıp içtiği belirlendi. Yoğun bakıma yatışı yapılan hastanın glaskow koma skalasının sekizin altında olması, solunumunun yüzeyel olması üzerine sedatize ve kürarize edilerek orotrakeal entübasyonu gerçekleştirildi. Nazogastrik yoldan %40'lık etanol verildi. Folik asit, tiamin ve %5 dekstroz tedavisine başlandı. Derin asidozu olan hastaya sodyum bikarbonat infüzyonu başlandı. Hasta dokuz gün sonra görme kaybı ile taburcu edildi
