Blood parameters in infants aged between 4 and 7 months who nourished with breast-feeding, formula milk or cow’s milk

Abstract
The effect of nutrition with breast-feeding and formula milk and cow’s milk on blood parameters and the body iron status were evaluated. Methods: Total 66 infants were included in study. The study was designed with three groups; in the group 1: 31 (47.0 %) infants who nourished exclusively breast feeding. Groups 2: 21 (31.8 %) infants who nourished exclusively formula milk and Groups 3: 14 (21.2 %) infants who nourished exclusively cow’s milk. We evaluated the blood parameters which are erythrocyte count [RBC] and hemoglobin [Hb] and hematocrit [Hct] and mean corpuscular volume [MCV] and mean corpuscular hemoglobin [MCH] and mean corpuscular hemoglobin concentration [MCHC] and serum iron [SI] and total iron binding capacity [TIBC] and ferritin [F] in all groups. The study group was composed from infants aged between 4-7 months who were feeding only one of these nutrients so that removing the efficacy diet and iron prophylaxis. All the groups was made infants who born on time and have no chronic disease finding. Infants who have another diagnosis such as infants with small for gestational age and preterm and thalassemia triats and megaloblastic anemia was excluded from the study. Results: Statistical difference between groups concerning the values of Hb, Hct, MCH, MCHC, SI and F was observed (p<0.05, Group 1-3: Hb, Hct, MCH, MCHC, SI ve F; Group 2-3: Hb, Hct, MCH, MCHC ve SI). 66 infant was included to study and 2 of them had (3 %) iron loosing and 5 of them had (7.5 %) iron deficiency and 13 of them had (19.6 %) iron deficiency anemia (IDA). The 31 infant in the Group 1; in 1 of them (% 3.2) iron loosing and in 3 of them (% 9.6) iron deficiency and in 3 of them (% 9.6) IDA was determined. The 21 infant in the Group 2; in 1 of them (% 4.7 %) iron loosing and in 2 of them (9.5 %) iron deficiency and in 2 of them (% 9.5) IDA and was determined. The 14 infant in the Group 3; in 8 of them (57.1 %) IDA was determined. Iron loosing iron deficiency and didn’t determine in this group. Conclusion: Between infants who are especially breast-feeding and cow’s milk were found markedly difference in point of hematological parameters and iron deficiency. Breast-feeding giving should be supported after the every delivery.

Anne sütü, formül süt ve inek sütü ile beslenmenin kan parametreleri ve vücut demir durumu üzerine etkileri belirlenmek istenildi. Yöntem: Bu amaçla 66 sütçocuğu değerlendirildi. Grup 1: Yalnızca anne sütü ile beslenen 31 (% 47.0), Grup 2: Yalnızca formül süt alan 21 (% 31.8) ve Grup 3: Yalnızca inek sütü ile beslenen 14 (% 21.2) sütçocuğundan oluşturuldu. Her bir grupta kan parametreleri (eritrosit sayısı [RBC], hemoglobin [Hb], hematokrit [Hct], ortalama eritrosit volümü [MCV], ortalama eritrosit hemoglobini [MCH], ortalama eritrosit hemoglobin konsantrasyonu [MCHC], serum demiri [SD], total demir bağlama kapasitesi [TDBK], ferritin [F]) değerlendirildi. Alınan besinler ve demir profilaksisinin etkisini ortadan kaldırmak için yalnızca bu üç tarz besini alan 4-7 ay arası sütçocukları çalışma kapsamına alındı. Bütün olgular zamanında doğan ve kronik bir hastalık bulgusu olmayanlardan oluşturuldu. Prematüre ve düşük doğum ağırlıklı bebekler, talasemi taşıyıcıları, megaloblastik anemi gibi başka tanılar almış bebekler çalışma dışı bırakıldı. Bulgular: Gruplar arasında Hb, Hct, MCH, MCHC, SD ve F düzeyleri bakımından farklılıklar saptandı (p<0.05, Grup 1-3: Hb, Hct, MCH, MCHC, SD ve F; Grup 2-3: Hb, Hct, MCH, MCHC ve SD). Çalışmaya alınan 66 süt çocuğundan 2 (% 3)’sinde demir azalması, 5 (% 7.5)’inde demir eksikliği ve 13 (% 19.6)’ünde demir eksikliği anemisi (DEA) saptandı. Grup 1’deki 31 sütçocuğundan 1 (% 3.2)’inde demir azalması, 3 (% 9.6)’ünde demir eksikliği ve 3 (% 9.6)’ünde DEA saptandı. Grup 2’deki 21 süt çocuğundan 1 (% 4.7)’inde demir azalması, 2 (% 9.5)’sinde demir eksikliği ve 2 (% 9.5)’sinde DEA saptandı. Grup 3’deki 14 sütçocuğundan 8 (% 57.1)’inde DEA belirlendi. Bu grupda demir azalması ve demir eksikliği saptanmadı. Sonuç: Özellikle anne sütü ve inek sütü alan sütçocukları arasında hematolojik değerler ve demir eksikliği görülmesi bakımından belirgin fark bulundu. Doğumdan sonra tüm süt çocuklarına anne sütü verilmesi desteklenmelidir.
