A review of indications, complications and outcome of percutaneous endoscopic gastrostomy (PEG) applications in children

Abstract

Aims: The experience on pediatric percutaneous endoscopic gastrostomy (PEG) applications is relatively limited in children in our country. An audit regarding PEG indications, complications, outcome and follow up is presented. Material and Methods: Beginning with the first case, a retrospective review of all children who underwent a PEG application was conducted by patient files, follow up records and telephone interviews for the most current data. Results: During a 44-month period, 26 PEG applications were done in 25 children. There were 11 males and 14 females. The median age was 76 months (range, 8 months-14 years). The most frequent indication for PEG insertion was chronic neurological disease (n=18) followed by intracranial tumors (n=6) and cleft palate (n=1). All procedures were done under general anesthesia in the operating suit. No early postoperative complications were encountered. Feeding through the catheter was initiated in the3rd postoperative day in all. The mean postoperative follow up was 13 months (range, 1,5-44 months). No procedure related deaths occurred; 10 children died of underlying disorders on an average of 8 months after PEG insertion. Among the remaining children, the catheter was removed in 2 after a mean of 9 months as the oral feeding was re-established. In one patient, a gastrocolic fistula was detected 4 months after PEG insertion. In an elective laparotomy, a Stamm gastrostomy was performed. In another one an open fundoplication with gastrostomy was done 5 months later. The remaining 11 children are alive and actively using the PEG catheters. Among these, one patient had a gross leakage at the gastrostomy site that cannot be managed conservatively. He underwent a second PEG insertion with removal of the first one. Conclusions: PEG offers the advantages of being a minimally invasive procedure, being easy to care for and enabling a permanent access for enteral nutrition in handicapped children. Regarding its simplicity, effectiveness and reproducibility, PEG should be the first-line method for providing long-term artificial enteral nutrition and definitely deserves every effort to be used more widely in our country.

Amaç: Ülkemizde çocuklarda perkütan endoskopik gastrostomi (PEG) uygulamalarına ilişkin deneyim oldukça sınırlıdır. Bu çalışmada, PEG endikasyonları, komplikasyonları, elde edilen uzun sonuçlar ve izlem bulguları derlenmiştir. Gereç ve Yöntem: İlk olgudan başlayarak, PEG uygulaması yapılan tüm olguların dosya kayıtları, izlem bulguları ve en güncel durumu değerlendirmek üzere yapılan telefon görüşmelerinde elde edilen bilgiler geriye dönük olarak değerlendirildi. Bulgular: 44 ay süresince 25 çocuğa 26 PEG uygulaması gerçekleştirildi. Hastaların 11’i kız ve 14’ü erkekti. Ortanca yaş 76 aydı (8 ay-14 yıl). En sık görülen PEG uygulama endikasyonu kronik nörolojik hastalıktı (n=18). Diğer endikasyonlar, intrakraniyal tümörler (n=6) ve yarık damaktı (n=1). Tüm girişimler genel anestezi altında ve ameliyathanede yapıldı. Erken ameliyat sonrasında komplikasyon görülmedi. Tüm olgular üçüncü günde kataterden beslenmeye başlandı. Ortalama ameliyat sonrası izlem süresi 13 aydı (1,5-44 ay). İşleme bağlı ölüm görülmedi. PEG uygulamasından ortalama 8 ay sonra, 10 hasta altta yatan hastalıklarına bağlı nedenlerle öldü. Diğer hastalardan ikisinde ağızdan beslenmeye geçilebilmesi üzerine, ortalama 9 ay sonra PEG katateri çıkarıldı. Bir diğer hastada PEG uygulamasından 4 ay sonra gastrokolonik fistül geliştiği tespit edilerek elektif koşullarda Stamm gastrostomi uygulandı. Bir diğer hastada PEG uygulamasından 5 ay sonra açık cerrahi ile fundoplikasyon ve gastrostomi yapıldı. Kalan 11 çocuk halen PEG kataterlerinden beslenmektedir. Bunların içinden bir hastada gastrostomi kenarından konservatif yöntemlerle başa çıkılamayan kaçak geliştiği için ilk katater çıkarılarak ikinci kez PEG uygulaması yapıldı. Diğer hastalarda majör komplikasyon görülmedi. Sonuç: PEG minimal girişimsel olması, bakımının kolaylığı ve engelli çocuklarda enteral beslenme için kalıcı bir yol sağlaması gibi avantajlara sahiptir. Basitliği, etkili olması ve tekrar uygulanabilirliği gibi nedenlerle uzun süreli yapay beslenme desteğini sağlamak konusunda PEG ilk tercih edilen yöntem olmalıdır. Bu nedenlerle ülkemizde de daha yaygın uygulanması için her türlü çabayı göstermeye değer bir seçenektir.
