A Case of Intraabdominal Undescended Testis

Abstract

In the early fetal period, the testes are a predominant endocrine gland localized behind the abdominal cavity. During the development of the testes, they migrate from the abdomen to the scrotum. The testes begin to descend through the canalis inguinalis at weeks 28, and this descent lasts 2-3 days. After 4 weeks (at 32 weeks), they descend into the scrotum by passing behind the peritoneum and processus vaginalis. During this migration, the testes can remain in the canalis inguinalis, the deep part of the canalis inguinalis, or the abdomen. The failure of the testes to descend can cause infertility. After the early childhood, the risk of testicular carcinoma in undescended testis increases. Early diagnosis and appropriate treatment can protect the person from problems that may arise in the future. In this study, we present a case of intraabdominal testis in a 50-year-old male patient who was admitted with the complaint of absence of right testis. We think that an accurate determination of the anatomical localization of the testis may be important for the patient's follow-up and planning of the surgical procedure to be performed.

Özet

Erken fetal periyotta testisler abdominal boşluğun arkasında lokalize olmuş predominant endokrin bir bezdir. Testislerin gelişimi sırasında abdomenden başlayıp scrotuma kadar uzanan bir göç söz konusudur. Testislerin canalis inguinalis içinden 28. haftada başlayan inişi 2 -3 gün sürer. 4 hafta sonra da (32. hafta) periton ve processus vaginalis arkasından geçerek scrotuma iner. Bu göç sırasında testisler canalis inguinalis’de, canalis inguinalis’in derininde ya da abdomende kalabilirler. Testislerin inişindeki geri kalış infertiliteye neden olabilir. Erken çocukluk döneminden sonra inmemiş testiste testiküler karsinoma riski artar. Tanının zamanında konularak uygun tedavinin yapılması kişiyi ileri dönemde ortaya çıkabilecek sorunlardan koruyabilmektedir. Bu çalışmada sağ testis yokluğu şikayeti ile başvuran 50 yaşındaki bir erkek hastada intraabdominal testis olgusu sunulmuştur. Testisin anatomik lokalizasyonlarının doğru olarak belirlenmesinin, hastanın takibi ve uygulanacak cerrahi girişimin planlanması açısından önemli olabileceğini düşünmekteyiz.
