Comparison of Peri-Operative Outcomes Between Hysteroscopic Morcellator and Resectoscopy for Endometrial Polypectomy Operation

Endometriyal Polipektomi Operasyonunda Histeroskopik Morselatör ve Rezektoskopun Peri-Operatif Sonuçlarının Kıyaslanması

Abstract

Objective: To compare the peri-operative outcomes of women who underwent hysteroscopic polypectomy with conventional resectoscope and hysteroscopic morcellation. Material and Methods: Retrospective data of the 83 patients who underwent hysteroscopic polyp resection with conventional resectoscope or hysteroscopic morcellator between June 2017 and April 2019 was analyzed for the study. Perioperative outcomes were compared between resectoscope and morcellator group. Results: Of the total 83 patients, 45 were underwent hysteroscopic polypectomy with conventional resectoscope, 38 were underwent hysteroscopic polypectomy with morcellator. Major characteristics of the patients in terms of age, Body mass index (BMI), parity, number of previous c-section and polyp size were not significantly different between groups. Operation time, number of hysteroscopic entry and fluid deficit were significantly lower in morcellator group than resectoscope group (p .05). Conclusion: Hysteroscopic morcellation for the treatment of uterine polyps decreases the operation time and fluid deficit when compared with the conventional hysteroscopic resectoscope resection, however it does not seem to change hospitalization time and post-operative pain.

Özet

Amaç: Konvansiyonel histeroskopik rezektoskop ve histeroskopik morselatör ile yapılan histeroskopik polipektomi ameliyatlarının perioperatif sonuçlarını kıyaslamaktır. Gereç ve Yöntemler: Haziran 2017- Nisan 2019 arasında rezektoskop veya morselatör ile histeroskopik polipektomi operasyonu geçirilen hastaların dataları retrospektif olarak analiz edildi. Sonuçlar: Toplam 83 hastanın, 45’ i konvansiyonel rezektoskop ile 38 ‘i morselatör ile histeroskopik polipektomi operasyonu geçirdi. Gruplar arasında temel karakteristikler olan yaş, vücut kitle indeksi (VKİ), parite, geçirilmiş sezeryan sayısı ve polip büyüklüğü açısından istatistiksel anlamlı fark bulunmadı (p>.05). Ameliyat süresi, histeroskopun giriş sayısı ve sıvı açığı morselatör kullanılan grupta konvansiyonel rezektoskop kullanılan gruba göre anlamlı daha az saptandı (p .05). Sonuç: Endometriyal polip tedavisi için yapılan histeroskopik morselatör ile rezeksiyon ameliyat zamanını, uterusa giriş sayısını ve sıvı açığını azaltmaktadır, ancak taburculuk zamanını ve ağrı şikayetini değiştirmemektedir.
