Effect of carbondioxide insufflation on bowel in an experimental NEC model

Abstract

Aim: The aim of the study is to evaluate the effects of laparoscopy, which is reported to be safely applied in newborns with NEC, on the gastrointestinal tract in a animal model. Material and Methods: Forty newborn Sprague-Dawley rats were divided into four different groups of ten. Group K was the control group with no intervention whereas Group P was subjected to pneumoperitoneum using carbon dioxide insufflation under 10 mmHg for 30 minutes followed by desufflation for 30 minutes on the fourth day of the study. Rats in Group N were NEC models established by the method described by Okur. Group NP was subjected to pneumoperitoneum-desufflation on the fourth day of the study as Group P in addition to the procedures applied on Group N. Subjects were decapitated on the fourth day and histopathological findings of the resected terminal ileum, and nitric oxide, glutathione and TBARS levels were investigated . Statistical evaluation of biochemical results were was performed using ANOVA and Tukey HSD tests, whereas Mann Whitney-U and Kruskal -Wallis tests were used for the evaluation of histopathological results. Results: TBARS levels were significantly higher in Group NP compared to Groups K and P. However, a significant difference was not determined between the other groups. NO levels were significantly higher in Group NP than in Group K. There was no significant difference between the other groups. When GSH levels were compared, Group NP was found to have significantly higher GSH levels than Groups K, P and N. While any significant difference was not found between Groups 1, and N. Additionally, GSH levels were significantly higher in Group N when compared with Groups K and P. Histopathological stages of Groups P, N and NP were found to be significantly higher, whereas no difference was observed among these groups. Conclusion: In the study, we demonstrated that carbondioxide pneumoperitoneum does not have a negative contributing effect on the tissue damage seen in NEC. The results of this animal model study support the safety of diagnostic laparascopy, a newly used method, in NEC.

uygulanabilirliği bildirilen laparoskopinin, deneysel bir modelde gastrointestinal sistem üzerindeki etkilerini araştırmaktır. Gereç ve Yöntem: Kırk adet Sprague-Dawley yenidoğan sıçan onlu 4 gruba ayrılmıştır. K grubu, işlem yapılmayan kontrol; L grubu, işlem yapılmadan 4. gün 10 mmHg basınçta 30 dk. karbondioksit, 30 dk. desüflasyon yapılan; N grubu modifiye Okur yöntemiyle NEK oluşturulan; NP grubu ise N grubundaki işlemlere ek olarak 4. gün P grubundaki gibi pnömoperitoneumdesüflasyon oluşturulan gruplar olarak belirlenmiştir. Denekler 4. gün dekapitasyonla sakrifiye edilmiş ve terminal ileum rezeke edilerek histopatolojik değişiklikler ile nitrik oksit, glutatyon ve TBARS düzeyleri incelenmiştir. Biyokimyasal incelemelerin istatistiksel değerlendirilmesi için ANOVA ve Tukey HSD, histopatolojik sonuçlar için Mann Whitney-U ve Kruskal Wallis testleri kullanılmıştır. Bulgular: TBARS değerleri karşılaştırıldığında, NP grubunun K ve P gruplarına göre anlamlı derecede yüksek olduğu, diğer gruplar arasında ise anlamlı bir farklılık olmadığı bulunmuştur. NO değerleri karşılaştırıldığında NP grubunun K grubuna göre anlamlı derecede yüksek olduğu, diğer gruplar arasında ise anlamlı bir farklılık olmadığı bulunmuştur. GSH değerleri karşılaştırıldığında NP grubunun K, P ve N gruplarına göre anlamlı derecede yüksek olduğu, N grubunun K ve L grubuna göre anlamlı derecede yüksek olduğu, K ve P grupları arasında ise anlamlı bir farklılık olmadığı bulunmuştur. P, N ve NP gruplarının histopatolojik evreleri kontrol grubuna göre anlamlı derecede yüksek bulunurken, bu grupların kendi aralarında bir farklılık saptanmamıştır. Sonuç: Çalışmada, CO2 pnömoperitoneumun NEK’teki doku hasarına ek bir olumsuz katkısı olmadığı gözlenmiştir. Deneysel hayvan modeli üzerinde yaptığımız bu çalışmanın sonuçları, klinikte kullanılmaya başlanan NEK’te laparoskopik tanı uygulamasının güvenilirliğini destekler niteliktedir.
