MIDHUMERAL BLOCKADE FOR NON-OBSTETRIC SURGERY DURING PREGNANCY

Abstract
Operations during pregnancy and the anesthesia applied during these operations are very important in terms of morbidity andmortality since they have effects on both the mother and the fetus. Application of regional block is highly preferable when consideringthe risk of hypoxemia and aspiration and the side effects they create both on the mother and the fetus. However, peripheral blocksare not as common as central blocks.A Twenty week pregnant, 28 year old, woman, 83 kg in weight, with a history of cesarean section under general anesthesiawas planned to have an operation due to the right distal humerus fracture of the gestational weight. The decision was made toblock the Midhumeral axles. A total of 40 cc 0.25% bupivakain solution was injected in equal dose around each nerve in order toprevent median, radial, unlar and tourniquet pain respectively with 2mA current. There was no complications and no additionalanalgesic medication was needed.Considering the physiological changes of the pregnant woman, we believe that the peripheral nerve block performed by anexperienced anesthesiologist may be more advantageous both for the safety of the pregnancy and for the fetus than applyinggeneral anesthesia

Gebelik süresince geçirilen operasyon ve uygulanan anestezi, hem anne hem de fetüs üzerine etkisi olduğundan morbidite vemortalite açısından çok önem taşımaktadır. Rejyonal blok uygulaması, yaşanabilecek hipoksemi ve aspirasyon riski göz önünealındığında fetüs ve anne üstüne yarattığı yan etkiler sebebiyle, tercih nedeni olmuştur. Ancak periferik bloklar santral bloklarkadar yaygın uygulanmamaktadır.Daha önce genel anestezi altında sezeryan operasyonu öyküsü olan 20 haftalık, 28 yaşında, 83 kg ağırlığındaki gebenin sağdistal humerus fraktürü nedeniyle operasyonu planlandı. Midhumeral aksiller blok yapılmasına karar verildi. Hastaya, 2mA'likakım yardımıyla sırasıyla medyan, radyal, ulnar ve turnike ağrısını engellemek amacıyla muskülokutanöz sinir lokalize edilerektoplamda 40 cc % 0,25 bupivakain solüsyonu her sinir etrafına eşit dozda enjekte edildi. Herhangi bir komplikasyon yaşanmadıve ek doz analjezik ilaç gereksinimi olmadı.Deneyimli bir anestezi uzmanı tarafından gerçekleştirilecek periferik sinir bloğunun gebe ve fetüs açısından, gebenin fizyolojik vefarmakolojik değişiklikleri göz önüne alındığında genel anesteziye kıyasla daha avantajlı olabileceği kanısındayız
