RETROSPECTIVE EVALUATION OF VERTEBROPLASTY AND PERCUTANEOUS BALLOON KYPHOPLASTY PROCEDURES PERFORMED UNDER LOCAL ANESTHESIA AND SEDOANALGESIA

Abstract

Objective: The aim of this study is to investigate the preoperative characteristics, anesthetic management and complications of thepatients who underwent percutaneous vertebroplasty and percutaneous balloon kyphoplasty under local anesthesia and sedoanalgesia.Method: Following ethics committee approval characteristics, pain scores, hemodynamic status, anesthetic drug requirement andcomplications of the patients were retrospectively evaluated.Results: Percutaneous vertebroplasty and percutaneous balloon kyphoplasty procedures were performed in 48 patients. The meanage was 77.45 ± 9.21 years and 81.25 % of patients had concomitant diseases. In all cases, 1% lidocaine for local anesthesia and midazolamfentanyl-propofol-ketamine based sedoanalgesia were administered. Mean induction time, discharge time and procedure time were2.85 ± 1.09 minutes, 47.02 ± 14.15 minutes and 21.79 ± 10.08 hours respectively. The mean VAS pain scores were 8.00 ± 1.14 and1.02 ± 0.75 (p < 0.0001) before intevention and during discharge respectively. During the procedure, disruptive movement developed in 3patients (6.25 %) and respiratory depression/ desaturation which required jaw thrust maneuver were developed in 2 (4.6 %) patients. Conclusion: Although these are minimally invasive interventions, being familiar with patient characteristics and being prepared forpotential complications of percutaneous vertebroplasty/percutaneous balloon kyphoplasty is essential for patient safety. Local anesthesiacombined with ketamine-propofol-midazolam-fentanyl-based sedation is effective and could be used as a safe alternative for these cases

Amaç: Bu çalışmanın amacı lokal anestezi ve sedoanaljezi altında perkütan vertebroplasti ve perkütan balon kifoplasti uygulananhastaların preoperatif özelliklerinin, anestezi yönetiminin ve komplikasyonların incelenmesidir.Yöntem: Etik kurul onayını takiben olguların özellikleri, ağrı skorları, hemodinamik durum, anestezik ilaç gereksinimi ve komplikasyonlarretrospektif olarak değerlendirildi.Bulgular: Kırk sekiz hastaya perkütan vertebroplasti ve perkütan balon kifoplasti uygulandı. Yaş ortalaması 77.45±9.21 idi ve hastaların% 81.25'i yandaş hastalığa sahipti. Tüm olgularda lokal anestezi için % 1'lik lidokain ve midazolam, fentanil, ketamin ve propofol bazlısedoanaljezi protokolü uygulandığı saptandı. Ortalama indüksiyon süresi 2.85±1.09 dakika, işlem süresi 47.02±14.15 dakika ve taburculuksüresi 21.79±10.08 saat idi.Ortalama VAS ağrı skorları girişim öncesinde 8.00±1.14 ve taburculuk sırasında 1.02±0.75 (p<0.0001) idi. Girişim sırasında 3 olguda(% 6.25) istenmeyen hareket oldu. İki hastada (% 4.16) çene kaldırma manevrasına gereksinim gösteren solunum depresyonu/desatürasyon gelişti. Sonuç: Minimal invaziv girişimler olmasına karşın, hasta özellikleri hakkında bilgi sahibi ve perkütan vertebroplasti/perkütan balonkifoplasti uygulanan hastalarda gelişebilecek komplikasyonlara hazırlıklı olmak hasta güvenliği açısından esas teşkil etmektedir. Bu olgularda,lokal anestezi ile kombine edilen midazolam-fentanil-ketamin-propofol bazlı sedoanaljezi etkin ve güvenli bir alternatif olabilir
