Midgut volvulus not associated with malrotation: Case presentation and literature review

Abstract

In this study, midgut volvulus case, one of the common causes of intestinal obstruction in neonates, was presented. The main distinguishing feature of the case was the absence of concomitant anomalies of rotation and fixation. A midgut volvulus without malrotation was detected during the operation of a 14 day- old- girl who applied to our clinic due to sudden onset of abdominal distention and bilious vomiting. Caecum was detected in the right lower quadrant and there were no Ladd bands. There were enlarged lymph nodes throughout the mesentery. No specific pathology was detected in lymph node biopsy. At the moment, the patient is at its ninth postoperative month and there are no clinical problems. We reviewed the literature on this subject and presented this case because it is a rare condition.

Özet

Bu çalışmada, yenidoğanlarda intestinal obstruksiyonun sık görülen nedenlerinden biri olan midgut volvulus olgusu sunuldu. Olgunun temel ayırt edici özelliği volvulusa rotasyon ve fiksasyon anomalilerinin eşlik etmemesi idi. Ani gelişen batın distansiyonu ve safralı kusma nedeni ile polikliniğimize başvuran on dört günlük kız bebeğin intestinal obstruksiyon ön tanısı ile yapılan ameliyatı sırasında malrotasyonun eşlik etmediği midgut volvulus saptandı. Çekum sağ alt kadranda saptandı ve Ladd bantları yoktu. Tüm mezenter boyunca büyümüş lenf nodları mevcuttu ve lenf nodu biyopsisinde spesifik patoloji saptanmadı. Ameliyat sonrası takibinin dokuzuncu ayında hasta sorunsuz seyretmektedir. Literatür derlemesi yaparak nadir görülmesi nedeni ile bu olguyu sunmak istedik.
