Rehabilitation of a Localized Scleroderma Patient with Rigid Contractures

Abstract
Scleroderma is a chronic connective tissue disease character- ized by skin and visceral involvement. Progressive skin involve- ment may cause severe joint contractures, muscle weakness, and restrictions in activities of daily living. In this case report, the rehabilitation process of a 33-year-old woman diagnosed with localized scleroderma is presented. She had localized sclerotic changes and severe joint contractures on her left upper extremity including the scapular region. Active range of motion (ROM), muscle, hand pinch, and grasp strengths were measured. The Disabilities of the Arm, Shoulder, and Hand (DASH) question- naire was used to determine upper extremity functionality. The main elements of this therapy protocol were heated modalities, active stretching and strengthening exercises, and a dynamic or- thosis to stretch the elbow contracture. She was supervised by a therapist three times a week. She also conducted a home exercise program and continued to use the orthosis at home. The assess- ment results obtained at the beginning and one month later were compared to decide on the effectiveness of the therapy. ROMs were improved in general. These gains were as high as 40° at el- bow extension and shoulder abduction. Despite slight decreases in pinch strengths, there was 5.6 kg increase in grip strength. A considerable improvement was observed in the DASH score. No change was observed in muscle strength. In this report, it is shown that conventional rehabilitation modalities supported with the use of a dynamic stretching orthosis could be effective in the correction of rigid contractures and could enhance func- tional status in localized scleroderma patients.

Skleroderma, cilt ve iç organ tutulumuyla karakterize bir kronik bağ dokusu hastalığıdır. İlerleyici cilt tutulumu eklemlerde yaygın kont- raktürlere, kas zayıflıklarına ve günlük yaşam aktivitelerinde belirgin kısıtlanmalara neden olabilir. Bu vaka raporunda lokalize skleroder- ma tanısı alan 33 yaşındaki bir kadın hastanın rehabilitasyon süreci anlatılmaktadır. Hastanın sol üst ekstremitesinde skapulayı da içine alan bir bölgede lokalize sklerotik değişiklikler ve buna bağlı ileri düzeyde eklem kontraktürleri vardı. Tutulum bölgesinde aktif ek- lem hareket açıklıkları (EHA), kas, el tutma ve kavrama kuvvetle- ri ölçüldü. Üst ekstremite fonksiyonellik seviyesini belirlemek için The Disabilities of the Arm, Shoulder and Hand (DASH) anketi kullanıldı. Terapinin temel unsurları nemli ısı uygulaması, aktif, germe ve güçlendirme egzersizleri ve dirsek fleksiyon kontraktürü için statik progresif germe ortezi kullanımıydı. Egzersizler haftada 3 defa terapist kontrolünde yaptırıldı. Hasta diğer günlerde ev egzersiz programı uyguladı ve dirsek ortezini kullanmaya devam etti. Tera- pinin etkinliğini belirlemek için başlangıçta ve 1 ay sonra yapılan değerlendirme sonuçları karşılaştırıldı. EHA’larında genel bir artış elde edildi. Bu artışlar omuz abdüksiyonu ve dirsek ekstansiyonunda 40 dereceye ulaştı. El tutma kuvvetlerinde bir miktar azalma görül- mesine karşın kavrama kuvveti 5, 6 kg arttı. DASH anketi puanında belirgin ilerleme görüldü. Bununla birlikte manuel kas testi sonuç- larında herhangi bir değişiklik gözlenmedi. Bu raporda, statik prog- resif germe ortezi kullanımıyla desteklenen geleneksel rehabilitasyon yöntemlerinin lokalize sklerodermalı hastalarda rijid kontraktürlerin düzeltilmesinde ve fonksiyonel durumun iyileştirilmesinde etkili olabileceği gösterilmiştir.
