CHARACTERISTICS AND RESULTS OF OUT OF HOSPITAL CARDIAC ARREST PATIENTS BROUGHT TO THE EMERGENCY SERVICE

Abstract

Objective: Out-of-hospital cardiac arrest (OHCA) is one of the leading causes of death and disability worldwide. Analysis andmanagement of the prearrest or arrest patients brought to the emergency service can help prevent the development of applications.The aim of this study is to identify the patients brought to our hospital’s emergency service in out of hospital cardiac arrest(OHCA) and analyze the management of these patients in the hospital setting. Method: The cases brought to the emergency department in our hospital with cardiac arrest between the years of 2014-2015were evaluated retrospectively. Over eighteen years of age, out of hospital cardiac arrest patients were included in the study.Postresuscitate Referral patients accepted from other hospitals were excluded.Results: Data were obtained from 91 patients diagnosed with OHCA in one year period. All of these patients were admitted tohospital with emergency medical service vehicle. The age of the patients ranged from 20 to 94 and it was determined that 64.8%of them was male. In the analysis of the cardiac rhythms at the beginning of the arrests; asystole (68.1 %) and pulseless electricalactivity (28.6 %) were the most frequent ones. The mean cardiopulmonary resuscitation time was 25 minutes Thirty fourpatients who performed cardiopulmonary resuscitation did not respond to resuscitation. Patients who responded to the resuscitationwere followed in the surgical intensive care and reanimation (71.9 %), coronary intensive care (21.1 %) and internal intensivecare units (7 %). Three of the patients who responded to cardiopulmonary resuscitation were discharged.Conclusion: As a result overall survival rate is low in OHCA patients. We believe that there is a need for new studies andstrategies to strengthen the chain of survival in these cases

Amaç: Hastane dışı kardiyak arrest (HDKA) dünya çapında ölüm ve sakatlığın önde gelen nedenlerinden biridir. Acil servisegetirilen prearrest veya arrest hastaların analizi ve yönetimi, önleme uygulamalarının gelişimine yardımcı olabilir. Bu çalışmanınamacı, hastanemiz acil servisine HDKA ile getirilen hastaların belirlenmesi ve bu hastalarda hastane ortamında ki yönetiminanaliz edilmesidir.Yöntem: 2014 ve 2015 yılları arasında hastanemiz acil servisine kardiyak arrestle getirilen hastaların dosyaları retrospektifolarak tarandı. On sekiz yaş üstü hastane dışı kardiyak arrest gelişen hastalar çalışmaya dahil edildi, diğer hastanelerden sevklekabul edilen postresüsite hastalar çalışma dışı bırakıldı.Bulgular: Veriler bir yıllık süreçte HDKA gelişen 91 hastadan elde edilmiştir. Bu hastaların tamamı acil sağlık hizmeti aracıiçinde hastaneye getirildi. Hastaların yaş aralığının 20 ile 94 arasında değiştiği ve % 64.8'nin erkek olduğu tespit edildi. Arrestinbaşlangıcındaki kardiyak ritimler incelendiğinde, en sık asistoli (% 68.1) ve nabızsız elektriksel aktivite (% 28.6) olduğu görüldü.Arresti takiben uygulanan kardiyopulmoner resüsitasyon süresi ortalama 25 dakika idi. Kardiyopulmoner resüsitasyon yapılan34 (% 37.4) hastada resüsitasyona yanıt alınamadı. Resüsitasyona yanıt alınan hastalar cerrahi yoğun bakım ve reanimasyon 41(% 71.9), koroner yoğun bakım 12 (% 21.1) ve dahiliye yoğun bakım ünitelerinde 7 (% 7) takip edildi. Kardiyopulmoner resüsitasyonayanıt alınan hastaların 3'ü (% 5.3) taburcu edildi.Sonuç: Sonuç olarak HDKA gelişen hastalarda genel sağ kalım oranı düşük olup bu olgularda hayatta kalma zincirinigüçlendirmek için yeni çalışma ve stratejilere ihtiyaç olduğunu düşünmekteyiz
