Anesthesia Experience of a Patient with Myotonic Dystrophy

Abstract
Myotonic dystrophy is the most frequently encountered entity among myotonic syndromes. Patients suffer from cardiac, gastrointestinal and endocrine system- related problems as well as skeletal and smooth muscle abnormalities. Anesthesia management of patients with myotonic dystrophies is important because of muscle weakness, impairment of respiratory and cardiac functions, and potential perioperative complications such as malignant hyperthermia and rhab- domyolysis. In this case report, our experience with spinal anesthesia in a patient with myotonic dystrophy planned for operation due to post-traumatic development of femur fracture is pre- sented in the light of current literature.
Özet
Miyotonik distrofi, miyotonik sendromlar içinde en sık karşılaşılandır. İskelet ve düz kaslarda anormalliklerle birlikte kardiyak, gastrointestinal ve endokrin sistemle ilgili sorunlar içerir. Myotonik distrofilerde, kas güçsüzlüğü, solunumsal ve kardiyak fonksiyonların etkilenmesi, malign hipertermi ve rabdomiyoliz gibi potansiyel perioperatif komplikasyonlar görülebilmesi nedeniyle anestezi yönetimi önemlidir. Bu sunumda travma sonrası gelişen femur fraktürü nede- niyle ameliyat planlanan miyotonik distrofi tanısı olan bir hastada uyguladığımız spinal anestezi deneyimimiz literatür bilgileri ışığında sunulmaktadır.
