Anesthesia Management in Patient with Chanarin-Dorfman Syndrome

Abstract

Objective: In this case, we present total intravenous anesthesia (TIVA) in a patient with the diagnosis of Chanarin-Dorfman Syndrome (CDS) who will be operated for bladder stone. Method: The patient had a history of esophageal variceal bleeding. Also an INR of 1.71 was detected. General anesthesia was considered when the patient refused regional anesthesia. TIVA was preferred because of isolated hepatotoxic effects of inhaled anesthetic agents. Results: TIVA was performed safely, no intraoperative complication was seen. No significant changes were observed in liver tests performed weekly during the first month. Conclusion: In rare cases such as CDS, we think that TIVA can be used safely for anesthesia.

Özet

Amaç: Biz bu olguda, Chanarin-Dorfman sendromu (CDS) tanısı olan mesane taşı nedeniyle opere edilecek bir hastada total intravenöz anestezi (TİVA) uygulamasını irdeledik. Yöntem: Hastada özofagus varis kanaması öyküsü mevcut idi. Ayrıca tetkiklerinde INR: 1.71 belirlendi. Hastanın rejyonal anesteziyi reddetmesiyle birlikte genel anestezi düşünüldü. İnhaler anestezik ajanların izole hepatotoksik etkileri nedeniyle olguda TİVA tercih edilmiştir. Bulgular: Hastada TİVA güvenle uygulanmış ameliyat sırasında komplikasyon görülmemiştir. Ameliyat sonrası bir aylık süreçte haftalık bakılan karaciğer testlerinde anlamlı değişiklik görülmemiştir. Sonuç: CDS gibi nadir görülen olgularda anestezi amacıyla TİVA uygulamasının güvenle kullanılabileceği düşüncesindeyiz.
