Abstract

Amaç: Allogreft ve alloplastlar, tatmin edici klinik sonuçları olan yeterli miktarda elde edilebilen greft materyalleridir. Bu çalışmada, atrofik mandibula kretine tünel tekniği ile uygulanan alloplast ve allogreft materyallerinin kemik gelişimine katkısının belirlenmesi ve karşılaştırılması amaçlanmıştır. Gereç ve Yöntem: Çalışmaya 14 tek taraflı mandibular posterior atrofik kreti olan hastalar dahil edildi. Hastalar rastgele 2 gruba ayrıldı. Bilgisayarlı tomografik (BT) görüntüler ameliyat öncesi hastalardan stentlerle alındı. Operasyon öncesi yatay ve dikey kemik tespit edildi. Ameliyat sırasında, mukoperiosteal flep ‘tünel tekniği’ ile kaldırıldı ve daha sonra her iki greft materyali yerleştirildi. Ameliyat sonrası her ay takip programları planlandı. 4. ay sonunda akrilik stent ile postoperatif ölçümler için BT görüntüleri alındı Bulgular: Cerrahi sonrası dönemde iyileşme her grup için sorunsuzdu. Her iki grupta kret yüksekliği ve genişliğinde istatistiksel olarak anlamlı bir artış vardı (p=0.005, p=0.016, p=0.011, p=0.001; p<0.05 (alloplast greft grubu), p=0.000; p<0.05 allogreft grubu). Allogreft grubundaki artış diğer gruba göre istatistiksel olarak daha anlamlı bulundu (p=0.01; p<0.05). Alloplast greft grubundaki hastalarda kemik hacminde anlamlı bir artış olmasına rağmen, klinik olarak kemik implant yerleştirme için uygun değildi, çünkü implant yerleştirme sırasında yeterli primer stabilite sağlanamadı. Sonuç: Her iki çalışma materyalinin de kret boyutlarındaki artışa katkıda bulunduğunu belirtmek mümkündür. Ancak alloplast greft grubunda elde edilen kemiğinin kalitesi implant rehabilitasyonu için yeterli değildi.

Özet

Background and Aim: Allografts and alloplasts are graft materials with satisfactory clinical outcomes that can be obtained in sufficient quantities. This report aims to determine and compare the contribution of alloplast and allograft materials applied to the atrophic mandibular crest with the tunnel technique. Materials and Methods: 14 patients with unilateral mandibular posterior atrophic crests were included, randomly divided into two groups. Computed tomographic images were taken preoperatively from patients with stents. Horizontal and vertical bone were determined preoperatively. During surgeries, the mucoperiosteal flap was elevated using the “tunnel technique,” then both graft materials were placed into the flaps. Follow-up programs were scheduled at each month after surgeries. Computed tomographic images were taken for postoperative measurements with acrylic stents at the end of the fourth month. Results: Postsurgical healing was uneventful for each group. There was a statistically significant increase in crest height and width in both groups (p=0.005, p=0.016, p=0.011, p=0.001; p<0.05 (alloplast graft group), p=0.000; p<0.05 allograft group, respectively).The allograft group had statistically significantly higher bone volume both horizontally and vertically (p=0.01; p<0.05). Although there was a significant increase in the amount of bone volume in patients in the alloplast graft group, clinically, the bone was not suitable for implant placement because adequate primary stability had not been achieved during implant placement. Conclusion: It is possible to state that both study materials contribute to the increase in crest dimensions. However, in the alloplast graft group, the quality of the remodeling bone was not adequate for implant rehabilitation.
