Seroprevelance of Hepatitis B Virus, Hepatitis C Virus and Human Immunodeficiency Virus in Northern Cyprus

Abstract
Objective: Epidemiological investigation of hepatitis B virus (HBV), hepatitis C virus (HCV) and human immunodeficiency virus (HIV), cause of high morbidity and mortalite, is important to define the spread and the protection of the viruses. The aim of the study was to determine the seroprevalance of HBV, HCV and HIV in North Cyprus and investigate the demographic information of the cases. Materials and Methods: The study group included the civillian-soldiers-public people, blood donors, tourists and people coming for education and/or work to North Cyprus and have to do these tests juridically and applied to do all and/or any two and/or any one of HBsAg, anti-HCV and anti-HIV tests between 2010-2014 in Near East University Hospital Microbiology Laboratory. HBsAg, anti-HCV and anti-HIV tests were determined chemiluminesans immunoassay (CEIA) (Architect i2000 SR, Abbott, USA) according to the manufactures. The cut off values of the tests were accepted (S/CO) ≤1 as positive. Results: Twenty-five thousond four hundred and forty-two people with 113 different nationality were analyzed in this study. Seventeen thousand five hundred and twenty-nine (68.9%) were male, 7.913 (31.1%) were women and the mean age was 34.32±14.24 (separation: 0-102). Out of 25.068 HBV cases, 339 (1.35%) were positive, this ratio was 31/24.973 (0.1%) and 9/24.044 (0.04%) for HCV and HIV respectively. Seroprevalances of Turkish Republic of Northern Cyprus citizens were obtained as HBsAg 0.8%, anti-HCV 0.014% and anti-HIV 0.03%. There is no statistically significant differences in terms of sex in HBsAg and HCV seropositivity but, all anti-HIV positive individuals were male (100%, n=9). Conclusion: The study results showed that Northern Cyprus with 300.000 population is in low endemicity countries according to HBV, HCV and HIV seroprevalance. This is because of widespreading hepatitis B vaccination programs, conscious society and legal obligation of outlanding of Turkish/foreign national HBV, HCV and HIV positive persons.

Amaç: Yüksek morbidite ve mortaliteye neden olan ve bulaşıcı hastalıkların ilk sıralarında yer alan hepatit B virüsü (HBV), hepatit C virüsü (HCV) ve İnsan immün yetmezlik virüsünün (HIV) epidemiyolojik açıdan incelenmesi, söz konusu virüslerin bulaş yollarının belirlenmesinde ve korunmada önem taşımaktadır. Bu çalışmada, Kuzey Kıbrıs Türk Cumhuriyeti’ndeki (K.K.T.C.) HBV, HCV ve HIV seroprevalansının belirlenmesi ve olguların demografik açıdan incelenmesi amaçlanmıştır. Gereç ve Yöntemler: Çalışma grubu, Yakın Doğu Üniversitesi Hastanesi Mikrobiyoloji Laboratuvarı’na 2010-2014 yılları arasında HBsAg, anti-HCV ve anti-HIV tarama testlerinin hepsini ve/veya herhangi ikisini ve/veya herhangi birini yaptırmak için başvuran sivil-askeri halk, kan donörleri, seyahat edenler ve ülkemize Türkiye ve diğer yabancı ülkelerden çalışmak, ikamet etmek ve öğrenim görmek amacıyla gelen ve yasal olarak söz konusu tarama testlerini yaptırmak zorunda olan kişilerden oluşmuştur. Çalışma grubundaki kişilerin HBsAg, anti-HCV ve anti-HIV testleri kemilüminesans enzim immünoassay yöntemiyle (Architect i2000 SR, Abbott, ABD) üretici firmanın önerileri doğrultusunda çalışılmıştır. Testlerin eşik değeri (S/CO) ≤1 üzerindeki sonuçlar pozitif olarak kabul edilmiştir. Bulgular: Çalışmada 113 farklı uyruktan 25,442 kişi incelenmiştir. Bunların 17,529’u erkek (%68,9), 7,913’ü kadın (%31,1), yaş ortalamaları ise 34,32±14,24 (aralık: 0-102) idi. HBsAg testini yaptırmak için başvuran 25,068 kişinin 339’unda (%1,35) pozitiflik saptanırken, anti-HCV ve anti-HIV için bu oranlar sırasıyla; 24,973 kişide 31 (%0,1) ve 24,044 kişide 9 (%0,04) şeklinde bulunmuştur. Yalnızca K.K.T.C. uyruklu vatandaşların söz konusu testlerdeki seropozitiflikleri ele alındığında, HBsAg %0,8, anti-HCV %0,014 ve anti-HIV %0,03 olarak tespit edilmiştir. Cinsiyet açısından HBsAg ve anti-HCV seropozitiflikleri karşılaştırıldığında istatistiksel olarak anlamlı bir fark bulunmazken, anti-HIV tarama testi pozitif bireylerin (%100, n=9) tümü erkekti. Sonuç: Araştırmamızın bulguları, yaklaşık 300,000 kişilik nüfusa sahip olan Kuzey Kıbrıs’ın, HBV, HCV ve HIV enfeksiyonu görülme sıklığı açısından düşük endemisiteli ülkeler arasına girdiğini göstermektedir. Bu durumu, yaygınlaşan hepatit B aşı programlarına, toplum bilincinin gelişmiş olmasına ve T.C./yabancı uyruklu HBV, HCV ve HIV pozitif kişilerin yasal zorunluluk nedeniyle yurtdışı edilmesine bağlamaktayız.
