TUR-P syndrome under spinal anesthesia: A case report

Abstract
TUR-P syndrome is a syndrome occurring during transurethral resection of prostate (TURP), and is characterised by a spectrum of symptoms which may range from asymptomatic hyponatremia to convulsions, coma and death. The consistence of syndrome depends on the amount of fluid that enters to the intravascular space through blood vessels in the resection zone. The first step in treatment is the control of bleeding and the termination of operation. In this report, a case of TUR-P syndrome caused by the absorbtion of a 27000 ml irrigation solution that used in surgery, under spinal anaesthesia in a 53 years old patient is discussed together with the recommendations for a clinical management and a possible treatment of acute hyponatremia.
Özet
Transüretral rezeksiyon (TUR-P) sendromu, prostatın transüretral rezeksiyonu sırasında ortaya çıkabilen; asemptomatik hiponatremiden, konvülzyon, koma ve ölüme kadar değişebilen semptomlarla karakterize komplikasyonlardandır. Sendromun ortaya çıkışı rezeksiyon bölgesindeki kan damarları ile dolaşıma giren sıvı miktarına bağlıdır. Tedavisinde ilk yapılması gereken kanama kontrolü ve operasyonun sonlandırılmasıdır. Bu sunuda, 53 yaşında spinal anestezi altında TUR yapılan ve cerrahi ekip tarafından kullanılan 27000 ml irrigasyon sıvısının absorbsiyonuna bağlı olarak TUR-P sendromu gelişen hastada, klinik yaklaşımımız ve akut hiponatremide olası tedavi yöntemleri tartışılmıştır.
