Early Morbidity and Mortality in Patients Undergoing Radical Gastrectomy and Bursectomy in Gastric Cancer

Abstract

Objective: Our objective in this study is to compare the preoperative parameters and postoperative early results of patients with gastric adenocarcinoma who were treated with radical gastrectomy and regional lymph node dissection technique and who underwent bursectomy additional to this technique. Materials and Methods: A total of 50 patients with gastric adenocarcinoma who underwent radical gastrectomy + D2 dissection (A) and radical gastrectomy + D2 dissection + bursectomy (B) in our clinic between January 2010 and January 2013 were evaluated retrospectively. Patients who underwent A and B surgeries were compared for their demographic characteristics, tumor histopathologies, perioperative status and postoperative complications within the first 30 days. Results: When demographic characteristics and tumor histopathologies of the patients were compared, no statistically significant difference was found between the 2 groups. When the patints’ perioperative status and postoperative complications within the first 30 days were compared, pancreatic leakage and pulmonary effusion were observed more frequently in the group with bursectomy; but the differences between the 2 groups were not statistically significant. Conclusion: By comparing the early results in our study, no differences were demonstrated in 2 groups in terms of complications and benefits. It is concluded that there is a need to evaluate the benefits and harms of bursectomy with more preoperative stating data and postoperative longer follow-up.

Özet

Amaç: Çalışmamızda mide adenokanseri nedeni ile radikal gastrektomi + bölgesel lenf nodu diseksiyonu tekniğini bu tekniğe bursektomi ekleyerek tedavi ettiğimiz hastaların preoperatif verilerini ve postoperatif erken dönem sonuçlarını iki grup arasında karşılaştırmaktır. Gereç ve Yöntemler: Ocak 2010-Ocak 2013 tarihleri arasında kliniğimizde mide adenokanseri nedeniyle radikal gastrektomi + D2 diseksiyon (A) ve radikal gastrektomi + D2 diseksiyon + bursektomi (B) yapılan toplam 50 hasta retrospektif olarak değerlendirilmiştir. A ve B ameliyatı yapılan hastalar demografik yapılarına, tümörün histopatolojisine, peroperatif ve postoperatif 30 gün içerisinde gelişen komplikasyonlara bakılarak karşılaştırıldı. Bulgular: Hastaların demografik verilerini ve tümörlerin histopatolojik özelliklerini karşılaştırdığımızda iki grup arasında istatistiksel olarak anlamlı fark bulunamadı. Yine hastaların peroperatif ve postoperatif ilk 30 gün içinde gelişen komplikasyonları karşılaştırıldığında bursektomi eklenen grupta pankreas kaçağı ve pulmoner efüzyon daha fazla görülmesine rağmen bu farklar istatistiksel olarak anlamlı değildi. Sonuç: Elimizdeki erken dönem sonuçların karşılaştırılmasıyla bursektominin komplikasyon ve fayda açısından bursektomi yapılmayan grupla farkı gösterilememiştir. Preoperatif daha çok evreleme verileri ile ve postoperatif daha uzun takiplerle bursektominin fayda ve zararının değerlendirilmesine ihtiyaç olduğu kanısına varılmıştır.
