Gemination and fusion are the most commonly encountered anomalies, and distinction between the two is always challenging. Gemination is defined as an attempt to make two teeth from one tooth bud. This case report describes the multidisiplinary treatment of a female patient with a geminated central tooth. The patient did not have history of anomaly in her primary dentition and in her family either. This type of dental anomaly can cause clinical problems in the form of malocclusion, poor aesthetic, and impaction of adjacent teeth caries, and periodontal destruction. For the treatment we preferred a new alternative method instead of hemisection surgery which is the gold standard for reducing the mesio-distal sizes of big crowns. Long term follow up is needed.
