Abstract

Laparoscopic cholecystectomy, being the initial treatment option for symptomatic biliary stone diseases, is one of the most common operations in general surgery clinics. Although the postoperative pain after laparoscopic cholecystectomy has been reported as decreased, it is still an important source of morbidity. The accumulated and non-propagated secretion in the lungs due to lack of coughing and deep respiration caused by severe pain after laparoscopic cholecystectomy might endup with some complications like atelectasis, pneumonia, and respiratory insufficiency. Here, we discuss a patient who developed bilateral atelectasis due to postoperative pain necessitating admission to intensive care unit and respiratory support cholecystectomy. after laparoscopic cholecystectomy

Özet

Laparaskopik kolesistektomi genel cerrahi kliniklerinde en sık yapılan ameliyatlardan birisidir ve özellikle son 20 yıldır semptomatik safra kesesi taşlarının tedavisinde ilk seçenek olmuştur. Postoperatif ağrının açık cerrahiye göre belirgin şekilde azaldığı bildirilmesine rağmen laparoskopik kolesistektomi sonrası halen önemli bir morbidite nedeni olmaya devam etmektedir. Ağrı nedeniyle hastanın öksürmemesi ve derin nefes alıp vermemesi sonucu akciğerlerde biriken ve atılamayan sekresyon postoperatif dönemde atelektazi, pnömoni ve solunum yetmezliği gibi pulmoner komplikasyonlara yol açabilir. Burada, laparaskopik kolesistektomi sonrasında postoperatif ağrıya bağlı bilateral atelektazi gelişen, solunum desteği ve yoğun bakım ihtiyacı gerektiren olgumuzu sunmaktayız.
