Abstract

Aim : To study the neuropathy prevalence and related situations in type 1 diabetics using the evaluation scales named Neuropathy Symptom Scale (NSS) and Neuropathy Injury Scoring (NIS). Method: 67 consecutive type 1 diabetic patients (female/male: 32/35) who were admitted to the outpatient clinic Diabetes at Dr Lütfi Kırdar Kartal Research and Training Hospital between January- March 2002; were taken into our study in order to evaluate the diabetic neuropathy by the same neurologist using NSS and NIS scales. Also, the diagnosis was confirmed in 28 cases by using EMG. On the other hand, routine biochemical parameters and levels of HbA1C of the cases were measured. Results: The mean chronological age of the cases was 27,1 ± 9,4 yrs, the mean age of the beginning of diabetes was 19,9 ± 8,2 yrs, and the mean duration of diabetes was 7,24 ± 6,8 yrs. 49 (74,1%) of patients had neuropathy according to the NSS scale while 42 (63,6%) had neuropathy when evaluated with NIS scale. We found impotency in 10 (20%) of 35 male patients in regard of autonomie neuropathy. 9 (13,6%) cases had incontinence. 4 (6%) had nocturnal diarrhea, 2 (3%) had signs of gastroparesis. We found signs of neuropathy in all of the 28 cases with EMG. We found statistically sinnificant correlation (p< 0,001) between the results of EMG and the two scoring system. Özet

Amaç: Tip 1 diabetlilerde Nöropati Semptom Skalası (NSS) ve Nöropati Hasarlanma Skoru (NHS) değerlendirme skalalarını kullanarak nöropati prevalansını ve bununla ilişkili durumları araştırmaktır. Metot. Ocak-Mart 2002 tarihlerinde Dr Lütfi Kırdar Kartal Eğ. ve Araş. Hastanesi diabet polikliniğine başvuran ardışık 67 Tip 1 diabetli (K/E32/35) vaka, aynı nörolog tarafından NSS ve NHS skalaları kullanılarak diabetik nöropati açısından değerlendirmek üzere çalışmaya alındı. Ayrıca 28 vakada EMG yapılarak tanı desteklendi. Diğer vakaların rutin biyokimya parametreleri ve HbAlC düzeyleri ölçüldü. Bulgular: Çalışmaya alınan vakaların kron. yaşı 27.1(9.4 yıl, diabet başlangıç yaşı 19.9(8.2 yıl, diabet süresi 7.24(6.8 yıl idi. HbA1C düzeyi % 9.4(2.6 olarak bulundu. NSS değerlendirmesine göre hastaların 49(%74.1)'unda nöropati varken, NHS değerlendirmesine göre 42(%63.6) hastada nöropati tespit edildi. Otonom nöropati açısından 35 erkek hastadan 10(%20)'unda impotans vardı. Vakaların 9(%13.6)'unda inkontinans, vardı. 4(%6)'ünde gece ishali, 2(%3)'sinde gastroparezi bulguları vardı.
