Abstract

Objective: Human brucellosis, a multisystem disease which may other conditions, has a low incidence in childhood and may easily be misdiagnosed. In children the most common route of infection is unpasteurized milk or milk products. Method: In the present study 27 children (20 boys and 7 girls) diagnosed as brucellosis were evaluated retros-pectively between 1997 and 1999 in Dr. Sami Ulus Children's Hospital. Results: The mean age was 9.3 years (range: 2 to 14 years). The most common manifestations were artbralgia .(81.5%) and fever (55%) followed by malaise (45%), headache (18.5%), myalgia (18.5%), anorexia (18.5%), vomiting (4%) and weight loss. (4%) hepatomegaly (33%) was found more often than splenomegaly (26%), arthritis (22%) and lymphadenopathy (4%). All patients had positive Rose Bengal slide agglutination tests and standard tube agglutination titres of 1/160 or more. All patients had negative blood cultures. Seven patients (26%) had no IgM response in 2-mercaptho ethanol test on initial evaluation. Children under 8 years were treated with co-trimoxazole (10 mg/kg/day trimethoprim, 2 doses) and gentamicin (5 mg/kg/day, 2 doses) and those over 8 years with doxycycline (4 mg/kg/day, 2 doses) and rifampicin (20 mg/kg/day, 2 doses), for at least 6 weeks. The treatment was well-tolerated and all patients responded by resolution of all symptoms within 4 weeks. Relapse occurred in two children all of whom responded to repeat therapy. Conclusion: Brucellosis should be considered in every child from an endemic area with a history of animal contact and presenting with a febrile illness.
