Abstract

Polyarteritis nodosa (PAN) is characterized by necrotizing inflammation ofsmall- or medium-sized muscular arteries. Atypic clinical presentation leadsto delay in diagnosis. For this reason, it can cause severe morbidity andmortality. Here, we described a case which presented first with fever and leftcostovertebral angle tenderness, and was misdiagnosed as pyelonephritis. The patient did not benefit from intravenous antibiotherapy that wasprescribed in the outpatient clinic and then hospitalized for left lowerabdominal pain. No source of fever was found despite detailed physicalexamination and laboratory investigation. An abdominal ultrasonographyshowed hyperechogenity of the left kidney. An abdominal computedtomography was performed for left lower abdominal pain suggestinginflammatory bowel disease findings. Colonoscopy showed an ulcero-vegetating and nodular mass surrounding the whole sigmoid colonmucosa and suggesting a malignant tumour. A conventional angiographyof the mesenteric artery showed total occlusion of the inferior mesentericartery. He developed acute abdominal pain and, left hemicolectomy wasperformed for colonic perforation. Histopathological findings consistentwith PAN were detected in the pathological examination of the resectedbowel, and the patient was diagnosed with PAN. After initiation ofcorticosteroid and cyclophosphamide therapy, the clinical status of thepatient was improved dramatically with disappearence of fever and reliefof abdominal pain.
