Abstract

Objective: To investigate the endometrial pathologies and make the differential diagnosis in women with postmenopausal bleeding. Material and Methods: We investigated the histopathological findings in women presenting postmenopausal bleeding in the menopause department of our hospital between March 2003 and March 2004. Results: The average age of 220 patients with postmenopausal bleeding was 53.1 (41-77). The frequencies of the main causes of the postmenopausal bleeding were : Proliferative endometrium in 49(22%), atrophic endometrium in 51 (23%), insufficient material in 97(44%), secretuary endometrium due to drug usage in 7(3,1%), endometrial polyp in 13 (6%), endometrial hyperplasia in 6 (2,7%) [4 (1,8%) of them without atypia and 2 (1%) of them with atypia], endometrial adenocarcinoma in 8 (3,6%), adenocarcinoma with squamous cell carcinoma in 3 (1,3%), chronic nonspesific endometritis in 5 (2,2%) patients and 1 (0,45%) case with indifferentiated malign tumor. And cervical polyps were diagnosed in 4 (1,8%) cases. Conclusions: Since there is risk of endometrial carcinoma, the women with postmenopausal bleeding must undergo D&C for exact diagnosis.

Özet

Amaç: Postmenopozal kanama ile başvuran hastalarda endometrial patolojilerin ortaya konması ve ayırıcı tanısının yapılması.Çalışmanın Yapıldığı Yer: SSK Ege Doğumevi ve Kadın Hastalıkları Eğitim Hastanesi Materyal ve Metod: SSK Ege Doğumevi ve Kadın Hastalıkları Eğitim Hastanesi Menopoz Polikliniğine Mart 2003-Mart 2004 tarihleri arasında başvuran postmenopozal kanaması olan hastalardan alınan endometrial biyopsi örneklerinin histopatolojik sonuçları değerlendirildi. Bulgular: Postmenopozal kanama şikayeti ile başvuran toplam 220 hastanın ortalama yaşı 53.1(41-77)’dir. 49’unda (%22) proliferatif endometrium, 51‘ inde (%23) atrofik endometrium, 97’sinde (%44) yetersiz materyal, 7’sinde (%3,1) ilaç kullanımına bağlı sekresyon fazında endometrium, 13’ünde (%6) endometrial polip, 4’ünde (%1,8) atipisiz ve 2’sinde (1%) atipili olmak üzere toplam 6’sında (%2,7) endometrial hiperplazi, 8’inde (%3,6) endometrial adenokarsinom, 3’ünde (%1,3) skuamöz hücreli karsinom ile beraber adenokarsinom, 5’inde (%2,2) kronik nonspesifik endometrit ve 1 (%0,45) hastada da indiferansiye malign tumör tespit edilmiştir. Ayrıca 4 olguda da (%1,8) servikal polip izlendi. Sonuçlar:Postmenopozal kanama şikayeti ile başvuran hastalarda endometriyum kanseri riski nedeni ile mutlaka endometrial biyopsi yapılarak patolojinin ortaya konması gereklidir.
