Abstract

This study aimed to evaluate pharmacokinetics, efficacy and tolerance of theophylline monohydrate "xanthium" in comparison to theophylline "talotren". Twenty five patients with chronic obstructive pulmonary disease enrolled in the study. Fifteen patients included in the group of xanthium were treated with 10 mg/kg theophylline once a day where as 10 patients included in the group of talotren were treated with theophylline twice a day. Clinical examination and pulmonary function testing were performed on the first and 28th days of the treatment. Plasma theophylline levels were assayed on the 7th day in 4 hour intervals to obtain the peak plasma levels (Cmax), the minimum plasma levels (Cmin) and, % fluctuations (Cmax-Cmin/Cmin X 100). Side effects were questioned on day 7 and day 28. In the group with xanthium; mean dose was 680 ± 126, mean plasma theophylline level was 10.6 ± 3.2 $mu$g/mL. Mean Cmax: 15.6 ± 8.3 u$mu$g/mL, Cmin: 5.0 ± 0.7 $mu$g/mL, mean 24 hour % fluctuation: 235.6 ± 201.2. In the group with talotren; mean dose was 650 ± 52.7 and, mean plasma theophylline level was 8.9±2.5 $mu$g/mL. Mean Cmax(T0-T12): 9.6 ± 2.9 $mu$g/mL, Cmin (T0-T12) 6.7 ± 2.3 $mu$g/mL, mean 12 hour (T0-T12) % fluctuation: 53.9 ± 64.8. % fluctuation rates were statistically significant between two groups (p< 0.01). Physical examination findings and pulmonary function testing were not statistically different in two groups. Ho severe side effects have occured in either group. These results indicate that talotren (bid) provides more stable plasma levels than xanthium (once a day). There is no advantage of one to the other in terms of efficacy and tolerance.
